Health (Shaviyani Atoll)
Basic medical care was introducedin the atoll with the establishmentofa health centre on25
December 1970 in the islandofLhaimagu.This healthcentre wasshifted to FarukolhuFunadhoo(now
Funadhoo) in1971 whenthe islandwasmade the atoll capital.This healthcentre is now developedto
Atoll Hospital whichhas a well equippedlaboratoryandoperationtheater facilitiesandare manned
by a gynecologist,anesthetistanda general practitioner.Other than Atoll Hospital,there are 13
Health Centre in the atoll with Doctor and Nurses.
Majorityof the populationofthe atoll have access to safe drinkingwater. Around99% of the
householdshavebeen providedwith water storagetanks on Tsunami recovery programm. Most
householdshavesafe sanitationfacilities.

UngoofaaruRegional Hospital providescurativeservices at the secondary level to the peopleof Raa.
Region andpreventive health programsthroughpublichealthunit andcontributes to the supervision
of inter atoll healthservices. In UngoofaaruRegional Hospital there are 116 staffsincluding
specialists,medical officers,registerednurses and other medical andadministrativestaffs.Available
services are providingadequatesecondary level care includingobstetricsandgynaecology,paediatric
care, internal medicine, dental care, surgery, physiotherapy,orthopedic,ophthalmology,X-ray,ultra
soundscanningandsome majoroperations.In addition,the hospital alsoprovidesemergency services
includingseaandland ambulanceservices,commonlaboratorydiagnosticfacilities including
microbiology.
The preventive healthservices are fall into two broadareas, namelyhealth promotionanddisease
control and surveillance.The programsconductingby the publichealthunit includesEPI program,
growth monitoring,school health,foodsafetyandreproductive healthincludingFamilyplanningand
counselingservices.

with the free aidof International HumanAssistancePrograms(IHAP) under their Integrated Rural

Health cares facilities are improving almost on a daily basis.
There are 21 hospitals all around the atolls.
The Indhira Gandhi Memorial Hospital (IGMH) in Malé is the biggest hospital in the country
providing sophisticated medical care.
ADK Hospital is the biggest private health care facility and follows high medical standards.
The total number of beds in the hospitals around the archipelago is around 800.There are 150 doctors
in Malé while 152 are in the rest of the country.
Some resorts have in-house doctors. A decompression chamber is within easy reach of most resorts in
case of a diving emergency.

Progress on MDGs in Maldives
Maldives is a low lying small island state in the Indian Ocean comprising of 1,190 islands, with a
population of 350,000 spread across the 194 inhabited islands. Despite our unique geography and
inherent vulnerabilities, the island nation of Maldives has, in the past four decades, shown
progress and incredible improvements in its health indicators. The country’s health system is
custom designed based on the size of the population and geographical location, offering targeted
levels of service. Every inhabited island has a primary health facility, while comprehensive
secondary level health services are made available within every atoll and tertiary level health
services available at the capital of the country. This structure, coupled with public health
programmes, has further facilitated the progress that Maldives has made in the health sector.
Future plans exist to develop regional tertiary facilities.
The resilient nature and commitment of the people of Maldives has placed Maldives as the only
‘MDG Plus’ country in South Asia, having achieved five out of the eight Millennium
Development Goals (MDGs) well ahead of the agreed timeline of 2015. Maldives is showing
potential to go beyond the MDG targets, especially in the health related MDGs 4, 5 and 6 to
address the reduction of child mortality, improvement of maternal health and combating of
HIV/AIDS, malaria and tuberculosis. Maldives also revels as the only country in the South-East
Asia region to eliminate malaria (in 1984) and to have sustained that achievement for 30 years. It
is worthy to state that remarkable progress has also been made towards the achievement of the
remaining three MDGs: MDG 3, achieving gender equality and women’s empowerment; MDG
7, ensuring environmental sustainability; and MDG 8, developing a global partnership for
development.
While we are proud of our achievements, sustaining them poses numerous challenges to the
Maldives. Geographical limitations, lack of economies of scale, heavy reliance on an expatriate
health workforce and dependence on imports of all medical consumables, pharmaceuticals and
other medical products has a profound effect on health expenditure, accessibility and availability
of quality health services within the country. The unique nature of Maldives also makes it
91. extremely susceptible to unprecedented climate change events that will unduly affect the already
vulnerable and marginalised (often women and children) and thereby intensify existing
inequalities.
Furthermore, these extreme weather events are set to impact on the incidence of non-
communicable diseases (NCDs), vector borne disease and basic conditions necessary for health
development, including access to safe drinking water, food security, shelter and sanitation
infrastructure.
The rapid progress in socioeconomic development and lifestyle changes has also prompted a
prominent transition to a NCDs morbid situation. Alarmingly, NCDs accounted for an estimated
78 per cent of all mortality in Maldives in 2012. The chronic disease burden has become a huge
challenge to health care provision and health expenditure. In today’s interconnected world,
Maldives is not and will not be free from risks of emerging and re-emerging communicable
diseases, and thus this area should remain a priority.
Despite the difficulties, the primary health care approach to health services, its commitment to
ensuring that the Maldivian people continue to enjoy good health and quality of life, has brought
systematic reforms to the health sector and enabled Maldives progress towards its health sector
achievements. The health system has been undergoing fundamental changes of reform, in
relation to legislative and policy aspects, and continues to evolve to meet emerging challenges.
Such reforms include the introduction of urgent and appropriate public health measures, with
special health programmes targeting adolescents and the prevention of cancer, chronic diseases
and non-communicable diseases. Strategies to improve utilisation of enabling platforms such as
telemedicine, e-health programmes and the initiatives for increased financial health protection
are of equal importance.
Besides these initiatives, Maldives is in dire need of new investments in health promotion,
disease prevention, infrastructure, technology and human resources, and availability of
medicines to make us better prepared to combat health challenges and continue growth in the
health sector.
There is also an urgent need to accelerate the strengthening of the health care system to meet the
increasing health care demands and the challenges of social determinants of health amidst
financial and resource shortages. Maldives’ position as a country graduating from ‘least
developed country’ status and its geography poses unique development challenges.
Nevertheless, Maldives hopes for a brighter future and an improved health care system if global
collective will and resources are dedicated towards this goal.
Hence, it is with this vision that Maldives wishes to engage in post-MDG framework discussions
and reiterate that health is not only crucial to sustainable development, but is a beneficiary of and
a contributor to development. Maldives supports the global aspiration towards universal health
coverage and control of NCDs at the heart of post-MDG discussions. It is our dearest hope that
the developments of sustainable development goals (SDGs) are viewed as a continuation of
MDGs and not in isolation with the progress that MDGs have made.
92. As a country that is extremely challenged by climate changes and unavoidable health risks, it is
our wish that SDGs and climate change are jointly addressed, keeping in mind that the links
between the two are central to reducing poverty, increasing economic security, enhancing social
development and achieving better health for all.
Child and maternal health in Maldives
Infant mortality in Maldives was nine deaths per 1,000 live births in 2012, with an under-five
mortality rate of 11 deaths per 1,000 live births. As shown in Graph 1, there has been a
consistent decline in the under-five mortality rate since 1990. This decline is impressive and has
seen the country exceed its target of 31 deaths per 1,000 live births, as defined by Millennium
Development Goal 4 (MDG 4). In 2010 the three most prominent causes of death for children
below the age of five years were congenital anomalies (30 per cent), prematurity (17 per cent)
and birth asphyxia (15 per cent). Other contributory causes were pneumonia (11 per cent),
neonatal sepsis (four per cent) and diarrhoea (three per cent). In the period 2007-11 Maldives had
a reported maternal mortality ratio of 140 deaths per 100,000 live births (this figure was
estimated at 60 deaths per 100,000 by UN agencies/World Bank in 2010).
Communicable diseases in Maldives
Communicable diseases along with maternal, perinatal and nutritional conditions in Maldives
accounted for an estimated 16 per cent of all mortality in 2008. Prevalence of HIV among people
aged 15-49 is 0.1 per cent (2012). Maldives eliminated malaria over the period 1973-1999.
Tuberculosis (TB) saw a significant decline in estimated incidence, which dropped by two-thirds,
and estimated mortality (when mortality data excludes cases co-morbid with HIV), which
dropped by around 90 per cent, in the period 1990-2012.
Non-communicable diseases in Maldives
Non-communicable diseases (NCDs) in Maldives accounted for an estimated 79 per cent of all
mortality in 2008. The most prevalent NCDs in Maldives are cardiovascular diseases, which
accounted for 44 per cent of total deaths across all age groups in 2008. Non-communicable
variants of respiratory diseases, cancers and diabetes contributed 14 per cent, eight per cent and
one per cent to total mortality respectively (2008).
Mental health in Maldives
There is a lack of information concerning the most commonly diagnosed mental illnesses in
Maldives. Neuropsychiatric disorders contributed an estimated 18.7 per cent of the global burden
of disease in 2008.

aldives’ public spending on health was 3.8 per cent of GDP in 2011, equivalent to US$545 per
capita. In the most recent survey, conducted between 1997 and 2010, there were 160 doctors, and
445 nurses and midwives per 100,000 people.
Additionally, in the period 2007-12, 95 per cent of births were attended by qualified health staff
and in 2012, 98 per cent of oneyear olds were immunised with one dose of measles. In 2011, 99
per cent of people were using an improved drinking water source and 98 per cent had access to
adequate sanitation facilities. The most recent survey, conducted in the period 2000-11, reports
that Maldives has 82 pharmaceutical personnel per 100,000 people.
Maldives has two hospitals on the main island of Malé and six regional hospitals. There are 13
atoll hospitals, 87 health centres, 51 family health sections and 37 health posts. The Ministry of
Health and Gender is the primary government ministry in charge of the sector, and has the remit
of of meeting the challenge of providing equitable services to a population scattered over many
islands. The reorganisation of the health system, with the introduction of atoll hospitals and
placement of doctors at health centres, has enabled increased access to medical services for the
island communities.
The logistical problems that had to be overcome in order to ensure effective provision of these
services included infrequent transport links and high operational costs. Accordingly, providing
health care services to the islands has almost doubled the cost of health services delivery. All
pharmaceutical products in Maldives are imported, mostly by the private sector. Apart from a
specific category of hospital drugs and controlled drugs, all drugs must be bought by patients at
private pharmacists.
There is no mental health act or plan, and mental health is not mentioned in general health
policy. There are 0.3 mental health outpatient facilities and no psychiatric hospitals or
psychiatric beds in general hospitals (2011).
Current health issues and progress in
Maldives
Maldives is currently working towards achieving the Millennium Development Goals. To
achieve the targets for the reduction of child mortality, which forms MDG 4, Maldives should
reduce underfive deaths per 1,000 live births to 31, and increase measles immunisation to 100
per cent by 2015. In 2012, under-five mortality stood at 11 deaths per 1,000 live births, and
measles immunisation at 98 per cent, so the country has already surpassed its under-five
mortality target and is on track to increase immunisation to 100 per cent by 2015. Consequently,
it is likely to achieve MDG 4.
The global MDG 5 target for maternal health is to reduce the number of women who die in
pregnancy and childbirth by threequarters between 1990 and 2015. When applying this target to
94. Maldives, maternal mortality should fall to 208 cases per 100,000 live births. In the period 2007-

target has already been surpassed. Part of the goal also stipulates that 100 per cent of births must
be attended by a skilled health professional. In the period 2007-12 this figure stood at 95 per
cent, so this target is on track to be achieved by 2015.
MDG 6 aims for a reduction in the prevalence of HIV, malaria and other diseases. Prevalence
rates of major communicable diseases such as malaria, HIV/AIDS and tuberculosis are low or
declining.
Accordingly, with continued progress, the country is likely to achieve MDG 6.
The 2013 World Health Organization (WHO) country co-operation strategy for Maldives reports
that Maldives has largely achieved all the health-related MDGs ahead of the 2015 deadline. The
WHO has, however, identified the rise of non-communicable diseases in the country as an issue
needing further attention. Noncommunicable diseases account for more than 70 per cent of all
mortality in the country. Both the obesity rate and the prevalence of low physical activity levels
across the population are among the highest in the WHO South-East Asia Region. Furthermore,
more than half of the adult population has high cholesterol, and more than a third of adult men
are daily tobacco smokers. In the most recent Maldives Health Master Plan, 2006-15, the
government acknowledged NCDs as one of the top health priorities. In January 2013 the Ban of
Smoking in Public Places regulation was implemented – it is the first tobacco control regulation
in the country and that shows steps are being taken by the government to tackle unhealthy
lifestyles that can contribute to the rise of NCDs.
95. Introducing Tree Top Hospital; the
new face of medical treatment at
Hulhumale’ in a future
Tree Top Health Pvt. Ltd. is set to build and operate a 159 bed hospital in Hulhumale’
under the name Tree Top Hospital.
After completion the hospital will be a tertiary level medical care centre ،with infrastructure and
standard of medical services comparable to commendable hospitals in South Asia and South East
Asia.

96. Tree Top Hospital will consist of 2 buildings ،with the hospital being housed in the main
building ،which will be having 6-storeys and a total floor area of approximately 20،000 square
meters. The second building of the hospital infrastructure will house support services ،such as
accommodation for staffs ،rooms for relatives or friends of patients ،restaurants and retail outlets.
This building will have 8-storeys and a total floor area of 10،000 square meters.
The target building completion date is slated at 11th November 2016 ،while the hospital will
become fully operational and receive patients around 1st March 2017.
