◦ Interdisciplinary team work is complex process in
which different types of health care profisionals work
together and collaboratively to share experience,
knowledge and skills, to impact on patient health
care.

5. Define interdisciplinary team
◦ Interdisciplinary team is used as a generic term
of reference for these health care teams which
included a range if health services workers both
professionals and non-professional with the
majority being from professional groups.

6. ◦ A dynamic process involving two or more health
professionas with complementary
backgrounds,skills,sharing common health goals and
exercising concerted physical and mental efforts in
assessing,planning or evalvating patient care.
◦ This accomplished through interdepedent collaboration,
open communication and shared descion making and
clear inter depndce interaction.

work
The need for interdisciplinary team work is increasing as a
result of a number of factors including
1. An aging population with frail older pepole and larger
numbers of patients with more complex needs
associated with chronic disease
2. The increasing complexity of skills and knowledge
required to provide comprehensive care to patients

3. Increasing specialization within health professions and
a corresponding fragmentation of disciplinary knowledge
resulting in health care professional being able to meet all
the complex needs their patients
4. The current emphasis in many countries policy
documents on multi professional team work and
development of shared learning

towards continous patient care quality improvement.
