Welcome to Open Enrollment 2012! <ul><li>All full time employees working ( 32+hrs) MUST COMPLETE THE 2012 ENROLLMENT FORM (yes, a form is required EVEN if you decide not to take the benefits) </li></ul><ul><ul><li>Your 2012 Enrollment form is found on MPG’s Intranet under the HR Department </li></ul></ul><ul><ul><ul><li>It’s EASY! Just follow the directions on the top of the form, SIGN and return! </li></ul></ul></ul><ul><ul><li>Complete this form and give to your site’s designated person </li></ul></ul><ul><ul><ul><li>To find out who your site’s designee is, go to the 2012 Benefit info on the intranet in HR department section or ask your Office Manager </li></ul></ul></ul><ul><li>Due Date Friday, November 18 th </li></ul>

Long-term Disability <ul><li>Did you know? </li></ul><ul><li>One out of every four people in today's work force will have a disabling condition of 90 days or greater before the age of 65 </li></ul><ul><li>You already have an individual LTD policy? Great! Your individual plan will supplement/stack with the group plan so you can get as much income as possible. </li></ul><ul><li>No medical questions or health exams required </li></ul><ul><li>Great group rates! </li></ul><ul><li>Pays up to Social Security normal retirement age </li></ul><ul><ul><li>If your annual salary is greater than $100k </li></ul></ul><ul><ul><ul><li>Own Occupation age, pays to age 65 </li></ul></ul></ul><ul><ul><ul><li>Pays 60% of base salary up to $10,000 per month </li></ul></ul></ul><ul><ul><li>If your annual salary is less than $100k </li></ul></ul><ul><ul><ul><li>Own Occupation (if you are able to return to work in a different profession), pays 24 months </li></ul></ul></ul><ul><ul><ul><li>Pays 60% of base salary up to $4,000 per month </li></ul></ul></ul><ul><ul><ul><li>****Separate application required </li></ul></ul></ul>**See plan summaries for details

Life Insurance <ul><li>Group Life Insurance </li></ul><ul><li>FREE </li></ul><ul><li>(for all Benefit Eligible Employees) </li></ul><ul><li>MPG has purchased </li></ul><ul><li>LIFE INSURANCE </li></ul><ul><li>for full time employees of </li></ul><ul><li>1 times your base salary </li></ul><ul><li>to a maximum of $200,000. </li></ul><ul><li>Voluntary Life Insurance </li></ul><ul><li>New group rates </li></ul><ul><li>Employee </li></ul><ul><li>– Available in $10,000 increments* </li></ul><ul><li>- Maximum is 5x salary, not to exceed $500,000 </li></ul><ul><li>- Guarantee issue is $50,000 </li></ul><ul><li>Spouse </li></ul><ul><li>– Available in $5,000 increments* </li></ul><ul><li>- Maximum is 50% of employee coverage not to exceed $250,000 </li></ul><ul><li>- Cannot be purchased without employee coverage </li></ul><ul><li>- Guarantee issue is $25,000 </li></ul><ul><li>Child </li></ul><ul><li>- Birth to 6 months, $500 </li></ul><ul><li>- Age 6 months to 30 years, $5,000 or $10,000 </li></ul><ul><li>- Maximum is 50% of employee coverage </li></ul><ul><li>- Cannot be purchased without employee coverage </li></ul><ul><li>***Separate application required </li></ul><ul><li>If you had MPG’s voluntary life insurance (vli) in 2011, your coverage amount will be grandfathered in.
