Yet while the engineers understand the technical aspects of PSM, there can still be formidable difficulties implementing the elements. In countries with high Power Distance and low Individualism, as in many developing economies, engineers look to line management for specific direction, setting of priorities, and resources. Let´s review what need to see to ensure our PSM is up and running

Successful PSM implementation requires either adaptations to the
local culture or change in the culture. Therefore, cultural differences
must first be recognized, followed by development of a PSM
implementation strategy that takes these differences into account.
Inability to culturally adapt the effort can result in an organization
that does not fully realize the benefits of Process Safety
Management.
3. Who is this for?
YourOrganization
WHY?: Key factors such as an aging workforce, a retirement boom in developed markets, a lack of skilled
workers in emerging markets and an increasing reliance on contractors have contributed to a significant
increase in operational risks. While most companies understand the need for a robust learning and
development program to build organizational capability and mitigate lack of awareness about Process Safety
Management, many lack a well defined method for transferring knowledge and developing the required skills.

Sense ofVulnerability
The Dunning–Kruger effect is a cognitive bias in which low-ability individuals suffer from illusory superiority,
mistakenly assessing their ability as much higher than it really is.
10. PSM is the proactive application of management principles to
a process for the prevention of loss of containment events
Any site which stores, handles or manufactures hazardous
substances or energy will have an inherent risk
Process Safety Events = Low Probability & High
Concequences
What is Process Safety Management
(PSM)?
“For a long time, people were saying that most accidents were due to human error and this is
true in a sense but it’s not very helpful. It’s a bit like saying that falls are due to gravity.”

How do you measure it?
12. Reliance on the low personal injury rate at Texas City as a safety indicator failed to provide a true picture of process safety performance and the health of the safety culture.
A “check the box” mentality was prevalent at Texas City, where personnel completed paperwork and checked off on safety policy and procedural requirements even when
those requirements had not been met.
BP Texas City lacked a reporting and learning culture. Personnel were not encouraged to report safety problems and some feared retaliation for doing so. The lessons from
incidents and nearmisses, therefore, were generally not captured or acted upon. Important relevant safety lessons from a British government investigation of incidents at
BP’s Grangemouth, Scotland, refinery

14. Assumption from this point on:
The approach that WXPERT always propose to companies is to adopt a Process Safety
Management based on Risk
For the purpose of this WEBINAR I am working with a RBPS approach of 20 Elements,
however my advise to Organizations is to be sure that you have the main pillars in place to
develop you proper elements in case you would not like to use the CCPS RBPS model
Main Pillars are:
1. Commitment to Process Safety (The most important one typically
treated one the system is already running)
2. Understanding of Hazard and Risks (Not many people understand
the difference)
3. Risk Management in Place (the complete process)
4. Learn from Experiences
Understanding the risk associated with an activity requires answering the following
questions:
1. What can go wrong?
2. How bad could it be?
3. How often might it happen?
Based upon the level of understanding of these answers, a company can decide what
actions, if any, are needed to eliminate, reduce, or control existing risk.

Operational Discipline, as we define it and most others would agree, is “doing the right
thing, the right way, every time.”That is a pretty simple definition, but when you start
thinking through the implications of what it takes for employees to be properly
equipped and have the motivation to do that, things get more complicated. If you
break down the definition, you see there are three areas of focus needed to drive the
right behaviors of Operational Discipline:
1. Know what the right thing to do is
2. Be willing to always do the right thing
3. Ensure others also always do the right thing
