ā€¢ When more than one fetus simultaneously develops in
the uterus then it is called multiple pregnancy.
ā€¢ Simultaneous development of two fetuses (twins) is the
commonest; although rare, development of three fetuses
(triplets), four fetuses (quadruplets), five fetuses
(quintuplets or six fetuses (sextuplets) may also occur.

ā€¢ If the division takes place within 72 hours after
fertilization the resulting embryos will have two separate
placenta, chorions and amnions (D/D)
ā€¢ If the division takes place between the 4th and 8th day
after the formation of inner cell mass when chorion has
already developed diamniotic monochorionic twins
develop (D/M)
ā€¢ If the division after 8th day of fertilization, when the
amniotic cavity has already formed, a monoamniotic
monochorionic twins develop (M/M)

ā€¢ On extreme rare occasions, division occurs after 2 weeks
of the development of embryonic disc resulting in the
formation of conjoined twins called-Siamese twins.
ā€¢ Four types of fusion may occur
ā€“ Thoracopagus (commonest)
ā€“ Pyopagus (Posterior fusion)
ā€“ Craniopagus (cephalic)
ā€“ Ischiopagus (caudal)

Each fetus is surrounded by a separate
amniotic sac with the chorionic layer
common to both.
Intervening membranes consist of 4
layers-amnion, chorion, chorion and
amnion.
Intervening membrane consists of two
layers of amnion only.

ā€¢ Sex: while twins having opposite sex are almost always
binovular and twins of the same sex are not always uniovular
but the uniovular twins are always of the same sex.
ā€¢ If the fetuses are of the same sex and have the same genetic
features (dominant blood groups), monozygosity is likely.
ā€¢ A test skin graft: Acceptance of reciprocal skin graftā€”proof of
monozygosity.
ā€¢ DNA microprobe technique is more definitive.
ā€¢ Follow-up study between 2-4 yearsā€”showing almost similar
physical and behavioral features suggestive of monozygosity.

Factors that Influence Twinning
Page 11
ā€¢ The causes of twin pregnancy is not known.
ā€¢ Race: Highest amongst Negroes (once in every 20 births),
lowest amongst Mongols and intermediate among Caucasians
ā€¢ Heredity: Family history in mother.
ā€¢ Maternal Age and Parity: Twinning peaks at age 37 years
ā€¢ Increasing parity: 5th gravid onwards.
ā€¢ Nutritional Factors: Taller, heavier womenā€”twinning rate 25 to
30 % greater.

Management
ā€¢ Antenatal diagnosis: ultrasound with doppler flow study
in the placental vascular bed.
ā€¢ Repeated amniocentesis to control polyhydramnios in
recipient twin.
ā€“ prevent preterm labour and placental abruption.
ā€¢ Selective reduction of one twin is done when survival of
both the fetuses is at risk.
ā€¢ Smaller twin generally have got better outcome.
ā€¢ Plethoric twin: risk of CCF and hydrops.
ā€¢ Perinatal mortality: 70%.

with poor
outcome of the Co-twin (25%) specially in monochorionic
placenta.
ā€¢ The surviving twin runs the risk of cerebral palsy,
microcephaly, renal cortical necrosis and DIC.
ā€¢ This is due to thromboplastin liberated from the dead
twin that crosses via placental anastomosis to the living
twin.

Antepartum Management of Twin
Pregnancy
Page 31
To reduce perinatal mortality and morbidity rates in
pregnancies complicated by twins, it is imperative that:
ā€¢ Delivery of markedly preterm neonates be prevented
ā€¢ Fetal-growth restriction be identified and afflicted fetuses
be delivered before they become moribund
ā€¢ Fetal trauma during labor and delivery be avoided, and
ā€¢ Expert neonatal care be available.

ā€¢ Diet: increased requirement of calories, protein, minerals,
vitamins, and essential fatty acids. Caloric should be
increased by another 300 kcal/day. Supplementation with 60
to 100 mg/day of iron and1 mg/day of folic acid.
ā€¢ Bed Rest
ā€¢ Antepartum Surveillance: sonographic examinations
ā€¢ Tests of Fetal Well-Being
ā€¢ Prevention of Preterm Delivery
ā€¢ Hospitalization
ā€¢ Use of corticosteroids to accelerate fetal lung maturation.

Delivery of the first baby:
ā€¢ Delivery: Same guidelines as in normal labour
liberal episiotomy.
ā€¢ Forceps delivery: if needed, should be done preferably
under pudendal block anaesthesia.
ā€¢ Do not give intravenous ergometrine with delivery of the
anterior shoulder of the first baby.
ā€¢ Clamp the cord at two places and cut it between.
ā€¢ At least 8-10 cm of cord is left behind for administration
of any drug or transfusion, if required.
ā€¢ The baby should be labeled one.

Conduction of labour after the delivery of the first baby:
Steps of management:
Step I:
ā€¢ Ascertain lie, presentation, size and FHS of the second
baby.
ā€¢ Vaginal examination: To confirm the abdominal findings
and to exclude cord prolapsed, if any to note the status
of membrane.

Lie longitudinal:
ā€¢ Step I: Low rupture of membranes, syntocinon, internal
examination to exclude cord prolapse.
ā€¢ Step II: If the uterine contraction is poor, 5 units of
oxytocin is added.
ā€¢ Step III: Is there is still a delay, interference is to be
done.

1. Vertex: Low downā€”forceps are applied.
ā€¢ High upā€”CPD should be ruled out.
ā€¢ The possibility of hydrocephalic head should also be
kept in mind and excluded by ultrasonography.
ā€¢ If these are excluded, internal version followed by breech
extraction is performed under general anesthesia.
ā€¢ Ventouse: effective alternative.

Multiple pregnancy chandni

  • 1. Powerpoint Templates Multiple Pregnancy Chandni Thampi Assistant professor Travancore college of nursing Page 1
  • 2. Powerpoint Templates Multiple pregnancy Page 2 ā€¢ When more than one fetus simultaneously develops in the uterus then it is called multiple pregnancy. ā€¢ Simultaneous development of two fetuses (twins) is the commonest; although rare, development of three fetuses (triplets), four fetuses (quadruplets), five fetuses (quintuplets or six fetuses (sextuplets) may also occur.
  • 3. Powerpoint Templates Twins pregnancy Page 3 (two-third) and Varieties: ā€¢ Dizygotic twins: is the commonest results from the fertilization of two ova. from theā€¢ Monozygotic twins (one-third) results fertilization of single ovum.
  • 4. Powerpoint Templates Genesis of twins ā€¢ Imonozygotic twins (syn. identical, uniovulvar) ā€¢ Dizygotic twins (syn: fraternal, binovular Page 4
  • 5. Powerpoint Templates On rare occasion, the following possibilities may occur Page 5 ā€¢ If the division takes place within 72 hours after fertilization the resulting embryos will have two separate placenta, chorions and amnions (D/D) ā€¢ If the division takes place between the 4th and 8th day after the formation of inner cell mass when chorion has already developed diamniotic monochorionic twins develop (D/M) ā€¢ If the division after 8th day of fertilization, when the amniotic cavity has already formed, a monoamniotic monochorionic twins develop (M/M)

Powerpoint Templates Page 9 Diamniotic Dichorionic Separate placenta Frequency: 35% Mortality: 13% Diamniotic DiChorionic fused placenta Frequency 27% Mortality 11% Diamniotic Monochorionic single placenta Frequency 36% Mortality 32% Monoamniotic Monochorionic single placenta Frequency 2% Mortality 44%
  • 8. Powerpoint Templates Page 10 Multiple pregnancy contdā€¦ ā€¢ On extreme rare occasions, division occurs after 2 weeks of the development of embryonic disc resulting in the formation of conjoined twins called-Siamese twins. ā€¢ Four types of fusion may occur ā€“ Thoracopagus (commonest) ā€“ Pyopagus (Posterior fusion) ā€“ Craniopagus (cephalic) ā€“ Ischiopagus (caudal)
  • 9. Powerpoint Templates Examination of placenta and membranes Page 9 Dizygotic Twin Monozygotic twin Two placenta, either completely separated or more commonly fused at the margin appearing to be one. No anastomosis between the two fetal vessels. Placenta is single. Varying degrees of anastomosis between the two fetal vessels. Each fetus is surrounded by a amnion and chorion Each fetus is surrounded by a separate amniotic sac with the chorionic layer common to both. Intervening membranes consist of 4 layers-amnion, chorion, chorion and amnion. Intervening membrane consists of two layers of amnion only.
  • 10. Powerpoint Templates ā€¢ Sex: while twins having opposite sex are almost always binovular and twins of the same sex are not always uniovular but the uniovular twins are always of the same sex. ā€¢ If the fetuses are of the same sex and have the same genetic features (dominant blood groups), monozygosity is likely. ā€¢ A test skin graft: Acceptance of reciprocal skin graftā€”proof of monozygosity. ā€¢ DNA microprobe technique is more definitive. ā€¢ Follow-up study between 2-4 yearsā€”showing almost similar physical and behavioral features suggestive of monozygosity. Page 10
  • 11. Powerpoint Templates Factors that Influence Twinning Page 11 ā€¢ The causes of twin pregnancy is not known. ā€¢ Race: Highest amongst Negroes (once in every 20 births), lowest amongst Mongols and intermediate among Caucasians ā€¢ Heredity: Family history in mother. ā€¢ Maternal Age and Parity: Twinning peaks at age 37 years ā€¢ Increasing parity: 5th gravid onwards. ā€¢ Nutritional Factors: Taller, heavier womenā€”twinning rate 25 to 30 % greater. ā€¢ Pituitary Gonadotropin ā€¢ Infertility Therapy ā€¢ Assisted Reproductive Technology
  • 12. Powerpoint Templates Terms Page 12 ā€¢ Superfecundation ā€¢ Superfetation ā€¢ Fetus papyraceous or compressus ā€¢ Fetus acardius ā€¢ Hydatidiform mole ā€¢ Vanishing twin
  • 13. Powerpoint Templates Diagnosis Page 13 History and Clinical Examination ā€¢ Recent administration of either clomiphene citrate or gonadotropins or pregnancy accomplished by ART are much stronger associates. ā€¢ Clinical examination with accurate measurement of fundal height.
  • 14. Powerpoint Templates Diagnosis contdā€¦ Page 14 ā€¢ In women with a uterus that appears large for gestational age, the following possibilities are considered: ā€“ Multiple fetuses ā€“ Elevation of the uterus by a distended bladder ā€“ Inaccurate menstrual history ā€“ Hydramnios ā€“ Hydatidiform mole ā€“ Uterine leiomyomas ā€“ A closely attached adnexal mass ā€“ Fetal macrosomia (late in pregnancy)
  • 15. Powerpoint Templates Diagnosis contdā€¦ of normal Page 15 pregnancy are often Symptoms ā€¢ Minor symptoms exaggerated. ā€¢ Increased nausea and vomiting in early months ā€¢ Cardio-respiratory embarrassment ā€¢ Tendency of swelling in the legs, varicose veins and hemorrhoids is greater ā€¢ Unusual rate of uterine enlargement and excessive fetal movements
  • 16. Powerpoint Templates Diagnosis contdā€¦ Page 16 explained by is a common General examination ā€¢ Prevalence of anemia is more ā€¢ Unusual weight gain, not preeclampsia or obesity ā€¢ Evidence of preeclampsia association.
  • 17. Powerpoint Templates Diagnosis contdā€¦ Page 17 Abdominal examination Inspection: Barrel shaped and the abdomen is unduly enlarged Palpation ā€“ Height of uterus > period of amenorrhoea ā€“ Girth of abdomen> normal average at term (100 cm) ā€“ Fetal bulk disproportionately larger in relation to the size of the fetal head. ā€“ Palpation of too many fetal parts ā€“ Finding of two fetal heads or three fetal poles Auscultation ā€¢ Two distinct FHS at separate spots, difference in heart rates is at least 10 beats/minute.
  • 18. Powerpoint Templates Diagnosis contdā€¦ Page 18 Investigations Sonography 10th ā€¢ separate gestational sacs identified early ā€¢ Confirmation of diagnosis as early as week of pregnancy ā€¢ Variability of fetuses, vanishing twin in second trimester ā€¢ Chorionicity (twin peak sign) ā€¢ Pregnancy dating, Fetal anomalies ā€¢ Fetal growth monitoring, Presentation and lie of fetuses ā€¢ Twin transfusion localization, Amniotic fluid volume
  • 20. Powerpoint Templates Diagnosis contdā€¦ Page 20 Biochemical Tests: ā€¢ Levels of hCG in plasma and in urine are higher ā€¢ Maternal serum alpha-fetoprotein level: Elevated ā€¢ Unconjugated oestriol: approximately double

  • 22. Powerpoint Templates Complications contdā€¦ ā€¢ During labour Early rupture of membranes and cord prolapse Prolonged labour Increased operative interference Bleeding Postpartum hemorrhage Page 22
  • 23. Powerpoint Templates Page 27 Complications contdā€¦ ā€¢ During puerperium Subinvolution Infection Lactation failure ā€¢ Fetal Miscarriage Prematurity (80%) Growth problem (25%) Intrauterine death Asphyxia and still birth Fetal anomalies
  • 24. Powerpoint Templates Complications of monochorionic twins Page 24 Twin twin transfusion syndrome (TTS) ā€¢ one twin appears to bleed into other through placental vascular anastomosis. ā€¢ Receptor twin becomes larger with hydramnios, polycythemic, hypertensive and hypervolemic ā€¢ Donor twin which become smaller with oligohydramnios, anemic, hypotensive and hypovolemic. ā€¢ Donor may appear stuck due to severe oligohydramnios. ā€¢ Difference of hemoglobin concentration between the twin usually exceeds 5 gm% and estimated fetal weight discrepancy is 25% or more.
  • 25. Powerpoint Templates Complications of monochorionic twins contdā€¦ Page 25 TTTS contd.. Management ā€¢ Antenatal diagnosis: ultrasound with doppler flow study in the placental vascular bed. ā€¢ Repeated amniocentesis to control polyhydramnios in recipient twin. ā€“ prevent preterm labour and placental abruption. ā€¢ Selective reduction of one twin is done when survival of both the fetuses is at risk. ā€¢ Smaller twin generally have got better outcome. ā€¢ Plethoric twin: risk of CCF and hydrops. ā€¢ Perinatal mortality: 70%.
  • 27. Powerpoint Templates Complications of monochorionic twins contdā€¦ Dead fetus syndrome ā€¢ Death of one twin (2-7%) is associated Page 27 with poor outcome of the Co-twin (25%) specially in monochorionic placenta. ā€¢ The surviving twin runs the risk of cerebral palsy, microcephaly, renal cortical necrosis and DIC. ā€¢ This is due to thromboplastin liberated from the dead twin that crosses via placental anastomosis to the living twin.
  • 28. Powerpoint Templates Complications of monochorionic twins contdā€¦ Page 28 Twin reversed arterial perfusion (TRAP): ā€¢ Characterized by an acardiac perfused twin having blood supply from a normal co-twin via large arterio-arterial anastomosis. Conjoint twin: high ā€¢ Rare. ā€¢ Perinatal survival depends upon the type of joint. ā€¢ Major cardiovascular anastomosis leads to mortality.
  • 30.
