The Bridge Model Evidence Base<br />The Bridge Model is an adaptation of the Enhanced Discharge Planning Program (EDPP)<br />EDPP is an evidence-based model developed and evaluated with a randomized-controlled trial at Rush University Medical Center (ITCC partner)<br />Bridge implements the evidence based components of EDPP and best practices developed by ITCC partner sites<br />Bridge is a hospital and community partnership<br />
