Population studies report a reduced (about 25% lower) reproductive capacity in cancer survivors than in the general population, regardless of the tumor type.
Men seeking infertility treatment had an increased risk of subsequently developing testicular cancer (standardized incidence ratio, 1.3; 95% confidence interval, 0.9- 1.9), with a markedly higher risk among those with known male factor infertility (2.8; 1.5-4.8). In multivariable analysis, men with male factor infertility were nearly 3 times more likely to develop testicular cancer compared with those without (hazard ratio, 2.8; 95% confidence interval, 1.3-6.0).
Men with most types of cancer have pretreatment semen parameters in the fertile range for density and in the intermediate range for motility. However, men with testicular cancer statistically have lower semen quality compared to those with other malignancies.
Sperm count, total sperm count, motility, and vitality decreased after treatment, with lowest values at 3 and 6 months. Twelve months after treatment, mean sperm count recovered to pretreatment values after doxorubicin, bleomycin, vinblastine, darcarbacine (ABVD) or ABVD þ radiotherapy, but not after doxorubicin, cyclophosphamide, vincristine, prednisone (CHOP) or mechlorethamine, oncovin, procarbazine, prednisone (MOPP) chemotherapies. It was noteworthy that 7% of patients remained azoospermic at 24 months. After 24 months, Kaplan-Meier estimates showed that more than 90% of patients will recover normal sperm count after ABVD or ABVD þ radiotherapy vs. 61% for CHOP chemotherapies.
The mean total sperm count recovered to pretreatment values at 12 months after treatment after two or fewer bleomycin, etoposide, and cisplatin (BEP) cycles, but not after radiotherapy or more than two BEP cycles.
In multivariable analysis, survivors had a decreased likelihood of siring or having a pregnancy versus siblings (male survivors: hazard ratio [HR] 0·63, 95% CI 0·58–0·68; p<0·0001) or of having a livebirth (male survivors: 0·63, 0·58–0·69; p<0·0001).
In male survivors, reduced likelihood of pregnancy was associated with upper tertile doses of cyclophosphamide (HR 0·60, 95% CI 0·51–0·71)
The mean sperm DFI in men with a diagnosis of cancer, 9.88% (95% confidence interval [CI] 7.84%–12.44%), did not differ from that found in men presenting for sperm donation 10.46% (95% CI 8.68%–11.80%). There were no significant differences in mean sperm DFI within cancer subgroups or when comparing testicular and nontesticular cancers.
Higher pre-treatment sperm DNA damage in TC patients in comparison with healthy, fertile controls but without significant differences between TC and infertile patients.
An overall significant decrease in post-treatment DFI (13.2% range 5.0 –70.5) compared with pretreatment values (17.1% range 5.1 –66.6) was found (P ¼ 0.040). In TGCT patients, post-treatment DFI was significantly higher in patients who were treated with radiotherapy (16.9% range 11.5 –39.9) compared with that in patients treated with chemotherapy (CT) alone (10.9% range 5.5– 39.9) (P ¼ 0.037). In HL patients, the type of treatment or number of CT cycles was not associated with DFI.
no increased risk of chromosomal abnormalities among survivors’ offspring was observed compared with their siblings’ offspring (odds ratio ¼ 0.99, 95% confidence interval ¼ 0.67 to 1.44, two-sided P ¼ .94), with similar risk between male and female survivors. Cancer survivors were not more likely than their siblings to have children with a chromosomal abnormality
For the CH group, there was a statistically significant decrease in sperm parameters, which was most significant 3 months after the end of chemotherapy. For the RT group, this decrease was most relevant 6 months after the end of radiotherapy. Two years after therapy, 3% of the CH group and 6% of the RT group remained azoospermic.
Experimental methods are being investigated including cryopreservation of immature testicular tissue, xenografting, and in vitro germ cell maturation.
Shetty and Meistrich hypothesized that the gonadotoxicity of cancer treatment led to significant increases in testosterone and FSH levels resulting in inhibition of spermatogonial differentiation. Therefore suppressing these levels using GnRH agonists or antagonists would potentially induce early recovery of spermatogonial differentiation
