Introduction
➢ The ‘Pharmacy and Therapeutic Committee’ (PTC) is a group of persons which
formulate policies regarding therapeutic use of drug.
➢ One of the methods or mode of ensuring the proper rationality in the use of
drugs is that the hospital organize and constitute,
“The Pharmacy and Therapeutic Committee”

Functions of PTC:
1. This committee should meet regularly at least six times in the year and also as and when
necessary.
2. Advising medical, administrative, and pharmacy departments on drug related issues.
3. Developing drug policies and procedures.
4. Evaluating and selecting drugs for the formulary and providing for its periodic revision.
5. Managing adverse drug reactions.
6. Managing medication error.

7. Criteria for inclusion of medicines on the formulary list (essential medicines list (EML))
8. Standard treatment guidelines and treatment algorithms, which should be the basis of
formulary selection
9. Generic substitution and therapeutic interchange
10. Information regarding new drugs which may have become commercially available.
11. Review of side effects, adverse drug reactions, toxic effects, drug interactions of drugs
12. Reports of various sub-committees.
PTC Role in Drug Safety:
➢ Drug safety is the moral, legal and professional obligation of pharmacist.
➢ There is a need to create safety awareness in all hospital departments.
➢ Following guidelines may sub serve the committee in ascertaining the adequate
safety factor of hospital pharmacy:
✓ The pharmacist should be aware of narcotic & psychotropic substances.
✓ The hospital must employ a qualified, at least, a registered pharmacist with at least B. Pharma
degree as ‘Chief Pharmacist’.
✓ Should not permit non-pharmacist personnel to dispense drugs and allied materials.
✓ Must provide adequate safe, work space, and storage facilities.
✓ Should have a drug formulary which periodically revised and kept up to date.
✓ The poisonous materials are separated from nonpoisonous materials in the pharmacy.
✓ Not permit non-pharmacist personnel.

PTC Role in ADR’S:
➢ There is a proportionate increase in the drug reactions. In order to gain an
understanding of these problems and to formulate competent opinions,
prevention and treatment, the PTC must assume the responsibility for the
developing and instituting a procedure for the purpose of committee.

➢ Every case of adverse drug reaction must be first reported by the attending
physician to the chairman of the PTC or clinical pharmacologist.
➢ The attending physician should complete the ‘Adverse Drug Reaction Report
form’ as illustrated above, on any patient having adverse reaction.
➢ The same is then ultimately recorded in the medical records.
➢ The PTC interacts with the various government bodies like- DTAB (Drugs
Technical Advisory Board), DCGI, AIIMS, PGI, Chandigarh for consultation.
➢ The medical record room will, upon the patients discharge, remove this report
from the medical record and forward it to the chairman, who in turn periodically
forward essential data to the central committee on Adverse Reactions formed by
the State Government or the drug control authorities of the state, Government
and the Drugs Controller or consultations with the bodies of experts such as
DTAB.

Role of PTC in Emergency Drug List :
➢ Emergency drugs always ready for severe condition in hospital department
admitted patients.
➢ It is absolutely necessary for the PTC of a hospital to get prepared boxes
containing emergency drugs which should be always available readily for use at
the bed-side.
➢ List of such drugs and other supplies should be complied by the committee, and
it should find their place in emergency kits.
➢ The emergency boxes always contains different emergency drugs like- digoxin,
amphetamine, atropine, WOI, amylnitrite etc.

Automatic Stop Order for Dangerous Drugs:
✓ All Drug Orders for narcotics, sedatives, hypnotic anticoagulants, and
antibiotics shall be automatically discontinued after 48 hours unless the order
indicates an exact number of doses to be administered, or the attending
physician, re-orders the medication.
✓ All orders for narcotics, sedative and hypnotics must be rewritten every 24
hours.
✓ In India, at present, this kind of system of issuing “ASODD” is not practiced
except for hospitals like Christian Medical Hospital Vellore or Jaslok Hospital,
Mumbai Escort group, Mayo Hospital etc.

  • 7. Agra Public Pharmacy College, Agra 7 Functions of PTC: 1. This committee should meet regularly at least six times in the year and also as and when necessary. 2. Advising medical, administrative, and pharmacy departments on drug related issues. 3. Developing drug policies and procedures. 4. Evaluating and selecting drugs for the formulary and providing for its periodic revision. 5. Managing adverse drug reactions. 6. Managing medication error.

Standard treatment guidelines and treatment algorithms, which should be the basis of formulary selection 9. Generic substitution and therapeutic interchange 10. Information regarding new drugs which may have become commercially available. 11. Review of side effects, adverse drug reactions, toxic effects, drug interactions of drugs 12. PTC Role in Drug Safety: ➢ Drug safety is the moral, legal and professional obligation of pharmacist. ➢ There is a need to create safety awareness in all hospital departments. ➢ Following guidelines may sub serve the committee in ascertaining the adequate safety factor of hospital pharmacy: ✓ The pharmacist should be aware of narcotic & psychotropic substances. ✓ The hospital must employ a qualified, at least, a registered pharmacist with at least B. Pharma degree as ‘Chief Pharmacist’. ✓ Should not permit non-pharmacist personnel to dispense drugs and allied materials. ✓ Must provide adequate safe, work space, and storage facilities. ✓ Should have a drug formulary which periodically revised and kept up to date. ✓ The poisonous materials are separated from nonpoisonous materials in the pharmacy. ✓ Not permit non-pharmacist personnel.   • 10. PTC Role in ADR’S: ➢ There is a proportionate increase in the drug reactions. In order to gain an understanding of these problems and to formulate competent opinions, prevention and treatment, the PTC must assume the responsibility for the developing and instituting a procedure for the purpose of committee.   • 12. ➢ Every case of adverse drug reaction must be first reported by the attending physician to the chairman of the PTC or clinical pharmacologist. ➢ The attending physician should complete the ‘Adverse Drug Reaction Report form’ as illustrated above, on any patient having adverse reaction. ➢ The same is then ultimately recorded in the medical records. ➢ The PTC interacts with the various government bodies like- DTAB (Drugs Technical Advisory Board), DCGI, AIIMS, PGI, Chandigarh for consultation. ➢ The medical record room will, upon the patients discharge, remove this report from the medical record and forward it to the chairman, who in turn periodically forward essential data to the central committee on Adverse Reactions formed by the State Government or the drug control authorities of the state, Government and the Drugs Controller or consultations with the bodies of experts such as DTAB. Role of PTC in Emergency Drug List : ➢ Emergency drugs always ready for severe condition in hospital department admitted patients. ➢ It is absolutely necessary for the PTC of a hospital to get prepared boxes containing emergency drugs which should be always available readily for use at the bed-side. ➢ List of such drugs and other supplies should be complied by the committee, and it should find their place in emergency kits. ➢ The emergency boxes always contains different emergency drugs like- digoxin, amphetamine, atropine, WOI, amylnitrite etc. Agra Public Pharmacy College, Agra 13

  • 15. Automatic Stop Order for Dangerous Drugs: ✓ All Drug Orders for narcotics, sedatives, hypnotic anticoagulants, and antibiotics shall be automatically discontinued after 48 hours unless the order indicates an exact number of doses to be administered, or the attending physician, re-orders the medication. ✓ All orders for narcotics, sedative and hypnotics must be rewritten every 24 hours. ✓ In India, at present, this kind of system of issuing “ASODD” is not practiced except for hospitals like Christian Medical Hospital Vellore or Jaslok Hospital, Mumbai Escort group, Mayo Hospital etc. Agra Public Pharmacy College, Agra 15
