Background-What is known?
• Candida blood stream infection (BSI) is an
important cause of neonatal sepsis and sepsis
related mortality.
• Common risk factors
– Prematurity
– very low birth weight (VLBW)
– central vascular catheterization
– parenteral nutrition
– use of
• broad-spectrum antibiotics
• H-2 blockers
• Corticosteroids
– endotracheal intubation
– prolonged hospital stay
Background-What is known?
• Although C. albicans accounts for 45–55%
of Candida BSI among infants, recent
studies have detected a shift towards non
albicans Candida (NAC) species.
• associated with high mortality and poor
antifungal susceptibility
What this Study adds?
• Non-albicans Candida (NAC) infection was
associated with higher mortality and
increased duration of hospital stay.
• Both C. parapsilosis and C. tropicalis
demonstrated higher resistance to
fluconazole and amphotericin B.
Objective
• To assess the epidemiology of
neonatal Candida blood stream
infection.
– the species distribution
– AFS pattern
– risk factors
– outcome of neonates
Methodology
• Type: Retrospective study
• Recruitment: Medical records of neonates with
Candida BSI over past 5 years (September 2010
to August 2015) were reviewed.
• Analysis: Clinical and investigation details,
treatments received, response to therapy and
outcome were noted.
Study Definition
• Candida BSI was defined as at least one pure
growth of Candida species in blood culture within
72 hours of inoculation, in presence of clinical
features suggestive of sepsis such as
– respiratory distress/apnoea tachycardia/bradycardia
– poor perfusion
– feeding intolerance
– temperature instability
– Lethargy
– seizures.
• Culture positivity within 14 days, with the same
Candida species was considered to be the same
infection episode.
Techniques and Materials
• Blood culture - paired samples were inoculated in sheep
brain heart infusion broth (Himedia, Mumbai, India) in 1:10
dilution and incubated at 37°C for 48 h.
• Any growth observed was subcultured on 5% sheep blood
agar, MacConkey’s agar, and Sabouraud’s dextrose agar
(SDA) with chloramphenicol (0.05%).
• Species was identified by colony morphology on SDA, colour
production on chromogenic media, growth at 45°C, germ tube
test, chlamydospore formation, and carbohydrate
fermentation and assimilation tests.
• Antifungal susceptibility was determined by the Clinical
Laboratory Standards Institute disk diffusion testing.
Statistical Analysis
• Analysis was done using SPSS 16.
• Risk factors were analysed by univariate
and stepwise multivariate logistic
regression analysis.

• C. Tropicalis, 32 (39%)
• C. albicans, 29 (35.4%)
• C. parapsilosis, 10 (12.2%)
• C. glabrata, 5 (6.1%)
• C. krusei, 4 (4.8%)
• C. guilliermondii, 2 (2.4%).

Drug Sensitivity
Susceptibility
• First line- Fluconazole ;
• Second line – Amphotericin B
• Change of antifungal chemotherapy was
based on clinical deterioration or
susceptibility testing.
• No antifungal chemoprophylaxis was given.
Susceptibility
• Voriconazole and caspofungin demonstrated 100%
AFS
• overall sensitivity for amphotericin-B, fluconazole
and itraconazole were 86.6%, 68.3% and 67.1%,
respectively
• C. parapsilosis and C. tropicalis demonstrated least
susceptibility to fluconazole and amphotericin-B
• 41.2% of Candida isolates from the neonates who
expired were resistant to both fluconazole and
amphotericin-B.
Conclusion
• C.tropicalis was the most commonly isolated
Candida species, followed by C. albicans and C.
parapsilosis.
• The incidence of mortality and duration of
hospital stay were significantly higher in NAC BSI.
• C. parapsilosis and C. tropicalis showed higher
resistance to fluconazole and amphotericin-B.
• High incidence of resistance to both fluconazole
and amphotericin B amongst infants who expired.
Strength of this study
• Observed a wide spectrum of multi-system
involvement, long-term complications and species
distribution.
• C. parapsilosis was identified as the most common
fungal species in neonates in earlier reports, which is
in contrast to our observation
• Compared to other Indian studies, mortality was less
in this study, but high incidence of resistance to both
fluconazole and amphotericin B amongst infants who
died was noted.
Limitations of this study
• Retrospective design
• Failure to perform species
identification in all cases.
Discussion
1. Prevention of risk factors in susceptible neonates
with early removal of central line,
2. Timely fungal culture,
3. Candida speciation and susceptibility testing
• These are necessary for appropriate institution of
treatment and better outcome.
• Frequent empirical use of fluconazole and
amphotericin B may be avoided as it may lead to a
shift in species distribution and higher antifungal
resistance.

Background-What is known? • Candida blood stream infection (BSI) is an important cause of neonatal sepsis and sepsis related mortality. • Common risk factors – Prematurity – very low birth weight (VLBW) – central vascular catheterization – parenteral nutrition – use of • broad-spectrum antibiotics • H-2 blockers • Corticosteroids – endotracheal intubation – prolonged hospital stay
  • 5. Background-What is known? • Although C. albicans accounts for 45–55% of Candida BSI among infants, recent studies have detected a shift towards non albicans Candida (NAC) species. • associated with high mortality and poor antifungal susceptibility
  • 6. What this Study adds? • Non-albicans Candida (NAC) infection was associated with higher mortality and increased duration of hospital stay. • Both C. parapsilosis and C. tropicalis demonstrated higher resistance to fluconazole and amphotericin B.
  • 7. Objective • To assess the epidemiology of neonatal Candida blood stream infection. – the species distribution – AFS pattern – risk factors – outcome of neonates
  • 8. Methodology • Type: Retrospective study • Recruitment: Medical records of neonates with Candida BSI over past 5 years (September 2010 to August 2015) were reviewed. • Analysis: Clinical and investigation details, treatments received, response to therapy and outcome were noted.
  • 9. Study Definition • Candida BSI was defined as at least one pure growth of Candida species in blood culture within 72 hours of inoculation, in presence of clinical features suggestive of sepsis such as – respiratory distress/apnoea tachycardia/bradycardia – poor perfusion – feeding intolerance – temperature instability – Lethargy – seizures. • Culture positivity within 14 days, with the same Candida species was considered to be the same infection episode.
  • 10. Techniques and Materials • Blood culture - paired samples were inoculated in sheep brain heart infusion broth (Himedia, Mumbai, India) in 1:10 dilution and incubated at 37°C for 48 h. • Any growth observed was subcultured on 5% sheep blood agar, MacConkey’s agar, and Sabouraud’s dextrose agar (SDA) with chloramphenicol (0.05%). • Species was identified by colony morphology on SDA, colour production on chromogenic media, growth at 45°C, germ tube test, chlamydospore formation, and carbohydrate fermentation and assimilation tests. • Antifungal susceptibility was determined by the Clinical Laboratory Standards Institute disk diffusion testing.
  • 11. Statistical Analysis • Analysis was done using SPSS 16. • Risk factors were analysed by univariate and stepwise multivariate logistic regression analysis.
  • 12. Out of 13,346 Neonates delivered Only 114 had Candidemia Species Identified only for 82 Sample Size 3128 were admitted in NICU

  • 20. Susceptibility • First line- Fluconazole ; • Second line – Amphotericin B • Change of antifungal chemotherapy was based on clinical deterioration or susceptibility testing. • No antifungal chemoprophylaxis was given.
  • 21. Susceptibility • Voriconazole and caspofungin demonstrated 100% AFS • overall sensitivity for amphotericin-B, fluconazole and itraconazole were 86.6%, 68.3% and 67.1%, respectively • C. parapsilosis and C. tropicalis demonstrated least susceptibility to fluconazole and amphotericin-B • 41.2% of Candida isolates from the neonates who expired were resistant to both fluconazole and amphotericin-B.
  • 22. Conclusion • C.tropicalis was the most commonly isolated Candida species, followed by C. albicans and C. parapsilosis. • The incidence of mortality and duration of hospital stay were significantly higher in NAC BSI. • C. parapsilosis and C. tropicalis showed higher resistance to fluconazole and amphotericin-B. • High incidence of resistance to both fluconazole and amphotericin B amongst infants who expired.
  • 23. Strength of this study • Observed a wide spectrum of multi-system involvement, long-term complications and species distribution. • C. parapsilosis was identified as the most common fungal species in neonates in earlier reports, which is in contrast to our observation • Compared to other Indian studies, mortality was less in this study, but high incidence of resistance to both fluconazole and amphotericin B amongst infants who died was noted.
  • 24. Limitations of this study • Retrospective design • Failure to perform species identification in all cases.
  • 25. Discussion 1. Prevention of risk factors in susceptible neonates with early removal of central line, 2. Timely fungal culture, 3. Candida speciation and susceptibility testing • These are necessary for appropriate institution of treatment and better outcome. • Frequent empirical use of fluconazole and amphotericin B may be avoided as it may lead to a shift in species distribution and higher antifungal resistance.
