26. * Ở người thừa cân: giảm 4,5 kg sẽ làm giảm HA, ngừa tăng HA.
* Giàu trái cây, rau quả, sản phẩm bơ sữa ít chất béo, giảm
cholesterol và mỡ (nhất là mở bảo hòa) giàu Kali và Calci.
* Vận động thể lực:
- Đi bộ nhanh > 30 phút /ngày trong hầu hết các ngày trong tuần.

* Nhất thiết phải khuyên bệnh nhân ngừng thuốc lá.

(không dồn vào cuối tuần rồi uống luôn 1 lần).

28. Sau vài tháng ngưng thuốc lá nguy cơ bệnh mạch vành giảm rõ rệt
và sau 3- 5 năm trở về giống như người chưa hút thuốc lá.
Sau 10 năm ngưng hút thuốc lá nguy cơ ung thư phổi giảm 30-
50%, sau 15 năm nguy cơ ung thư phổi giống như người chưa hút
thuốc lá.
Ngưng hút thuốc lá còn làm giảm nguy cơ ung thư khác như ung
thư thanh quản, thực quản, tuỵ tạng, bàng quang.
Ngưng hút thuốc lá còn làm giảm các bệnh lý khác như tai biến
mạch máu não, tắc mạch máu ở chân, bệnh phổi tắc nghẽn mãn tính,
viêm phổi và cải thiện được tình trạng khí phế thủng, viêm phế
quản.
Ở bệnh nhân tiểu đường ngưng hút thuốc lá làm giảm biến chứng
tim mạch và tai biến mạch máu não.
29. Uống rượu cũng có nhiều vấn đề cần quan tâm
1. Uống rượu quá nhiều, thường xuyên say xỉn rất dễ
bị đột quỵ,tai biến mạch máu não.
2. Có sự tỉ lệ thuận giữa bệnh tăng huyết áp và sự
uống rượu.
3. Người đang uống nhiều thường xuyên khi ngừng
rượu khi ngừng rượu đột ngột huyết áp có thể tăng vọt lên
và gây tai biến. Do đó nếu bạn đang nghiện rượu khi bỏ
rượu cũng phải giảm từ từ trong 3 tháng đến mức độ có thể
chấp nhận được.
4. Lượng rượu bia tối đa mà bạn có thể uống mà
không gây ảnh hưởng đến tim mạch là không quá 2 lon bia
hay 60ml rượu mạnh hoặc1/4 xị rượu đế một ngày.

CƠ SỞ CỦA VẤN ĐỀ:
1. Cơ chế bệnh sinh của tăng huyết áp là cơ chế đa yếu tố.

kích thích cơ chế phản-điều hòa (counter-regulatory)
3. Huyết áp kiểm soát được hiệu quả chỉ thấy được ở 50%
bệnh nhân với đơn trị liệu, trong khi đó phối hợp trị liệu có
thể đạt được kết quả đáp ứng trên 80% bệnh nhân.
4. Mức huyết áp đích rất khó đạt được với đơn trị liệu đặc
biệt trên bệnh nhân đái tháo đường và đã có tổn thương
cơ quan đích.

Open the session with discussion by posing a questions such as “How does high blood pressure make people feel?”
Discuss that for many people they will not “feel” high blood pressure. For most, it is a silent condition and a health risk. It is sometimes referred to as the “silent killer”.
Talking points:
Hypertension is a chronic condition and it is a serious condition that needs to be treated.
It is the most common chronic condition. In Canada, about 1 person in every 5 has high blood pressure, that is a blood pressure higher than 140/90 mm Hg. About 66% are treated and controlled.
Hypertension is more common among black people and those of South Asian background.

Target-organ damage precedes clinical events
The CV continuum begins with the well-known cardiovascular risk factors such as smoking and diabetes, initiating the process that leads to target-organ damage. The subclinical disease that starts as endothelial dysfunction, and the resultant oxidative stress and inflammatory response, gradually leads to vascular remodelling and atherosclerosis. Vascular remodelling in small, resistance arteries may be the initial step in the progression from hypertension to target-organ damage.
Small-artery remodelling is more common in patients with hypertension; increased blood pressure promotes the development of atherosclerotic plaques and in hypertensive vessels there is an increased expression of matrix and other proteins, and growth factors that can act to cause structural changes.
Cardiac remodelling is mediated by the diverse endocrine, paracrine and autocrine effects of a number of different hormones involved in changing the structure, function and phenotype of the myocardium. These include angiotensin II, cytokines and NO. If uninterrupted, cardiac remodelling results in impaired systolic and diastolic functioning and progresses to heart failure, end-stage heart disease and death. 1
We now consider that each disease event in the CV continuum is the result of common pathophysiological processes participating in multiple steps across the continuum.
The resultant atherosclerotic events and artery remodelling can be accompanied by cognitive dysfunction and, as the disease progresses, dementia2 and cardiac embolism can result in stroke – or can result in damage to the glomeruli, microalbuminuria and macroproteinuria, leading to progressive nephrosis and the development of renal failure.3–5

Treatment is a combination of pharmacotherapy, lifestyle factors and self management education.
All three should be initiated concurrently and sustained to ensure long term management of blood pressure at target levels.

It is likely that people with diabetes will require three or more medications to achieve and sustain target BP levels.

More than 1 BP reading is needed to make a diagnosis,
If the first reading is above the target level, two more readings should be taken during the same visit,
The first reading is discarded and an average is taken for the other two readings.

Persons with diabetes mellitus should be treated to attain systolic blood pressures of less than 130 mmHg (Grade C) and diastolic blood pressures of less than 80 mmHg (Grade A). (These target blood pressure levels are the same as the blood pressure treatment thresholds.) Combination therapy using two first-line agents may also be considered as initial treatment of hypertension (Grade B) if systolic blood pressure is 20 mmHg above target or if diastolic blood pressure is 10 mmHg above target. However, caution should be exercised in patients in whom a substantial fall in blood pressure is more likely or poorly tolerated (eg, elderly patients and patients with autonomic neuropathy).
For persons with diabetes and normal urinary albumin excretion (ACR of less than 2.0 mg/mmol in men and less than 2.8 mg/mmol in women) and without chronic kidney disease, with blood pressures of 130/80 mmHg or higher despite lifestyle interventions, any one of the following are recommended: an ACE inhibitor (Grade A for persons aged 55 years of age or older, and Grade B for persons younger than 55 years), ARB (Grade A for persons with left ventricular hypertrophy and 55 years of age or older, and Grade B for persons without left ventricular hypertrophy irrespective of age), dihydropyridine CCB (Grade A for persons younger than 55 years of age, and Grade B for persons younger than 55 years of age), or thiazide or thiazide-like diuretic (Grade A for persons 55 years of age or older, and Grade B for persons younger than 55 years), with special consideration to the ACE inhibitor and ARB, given their additional renal benefits. If these drugs are contraindicated or cannot be tolerated, a cardioselective beta-blocker (Grade B) or nondihydropyridine CCB (Grade B) can be substituted. Additional antihypertensive drugs should be used if target blood pressure levels are not achieved with standard-dose monotherapy (Grade B). The combination of an ACE inhibitor and ARB is not recommended in patients with diabetes and normal urinary albumin levels (Grade B).
For persons with diabetes and albuminuria (persistent ACR greater than 2.0 mg/mmol in men or greater than 2.8 mg/mmol in women), an ACE inhibitor or an ARB is recommended as initial therapy (Grade A). If blood pressure remains at 130/80 mmHg or higher despite lifestyle interventions and the use of an ACE inhibitor or ARB, additional antihypertensive drugs should be used to obtain target blood pressure.
For persons with diabetes and a normal urinary albumin excretion rate (ACR less than 2.0 mg/mmol in men or less than 2.8 mg/mmol in women) with no chronic kidney disease and with isolated systolic hypertension, a long-acting dihydropyridine CCB (Grade C) is an alternative initial choice to an ACE inhibitor (Grade B), ARB (Grade B), or a thiazide or thiazide-like diuretic (Grade C).
Alpha-blockers are not recommended as first-line agents for the treatment of hypertension in persons with diabetes (Grade A).
