3. PASSPORT MEANING
An official document issued by the government of a country to
one of its citizens and, varying from country to country,
authorizing travel to foreign countries and authenticating the
bearer's identity, citizenship right to protection while abroad,
and right to reenter his or her native country.

4. TYPES OF PASSPORT
Ordinary passport (Tourist passport, Regular passport,

5. Eligibility to apply for passport:

(a) Proof of address
(b) Proof of Date of Birth (attach one of the following):
(c) Citizenship Document if applicant is a citizen of India by
Registration or Naturalization.
(d) Government/Public Sector/Statutory body employees should
submit “Identity Certificate” in original
(e) If the applicant is eligible for “ECNR” attach attested copy of
supporting document.
6. Procedure for applying for Fresh
passport
a) Fill-in Form 1
b)Fill-in two personal particulars forms; if you have stayed
more than at one address during the last one year, fill-in those
many set of personal particular forms 1.
c) Provide proof of residence; Pl. submit one more proof of
residence if ration card is provided.
d) Provide proof of date of birth (Those who born after 26-01-
1989 only Birth Certificate from competent authority).
e) For ECNR – provide proof of 10th pass or any one document
as given under Section-III Column 15 in the Passport
Information Booklet.
If you are a Government/Public Sector Undertaking/Statutory
Body Employee, you need to provide two additional
documents:
1. Identity Certificate or NOC as per Annexure-B.
2. Notarised Affidavit as per Annexure-I.
7. To get the passport application
form:
The applicants can get the passport application form
directly from respective passport office/ District
Passport cells and in post office.
9. How to fill the form:
Use capital letters only
Use black/blue ball pen only
Do not fill the form with pencils or ink-pen
While filling up the boxes , kindly leave one box blank after
each completed word
Write clearly within the box without touching the boundaries
Adjust the information to fit within the number of given boxes
Do not write anything outside the box
Incomplete application will not be accepted
Self –attested copies are required
Illiterate applicants should put only thumb impression
10. Where to apply the form
Submit the duly filled in Passport Application Form in the
concerned passport Office or in the District Passport Centers
(DPCs) or in the Speed Post centers.

11. Fees Structure
The following all-inclusive fee is to be paid along with the application form, either by Bank
Draft in favor of the Passport Officer concerned or in cash.
1 Fresh Passport (36 pages) of 10 years Rs 1,000/-
validity
(including minors between 15 to 18
years of age, who wish to get a 10
years full validity passport)
2 Fresh Passport (60 pages) of 10 years Rs. 1,500/-
validity
3 Fresh Passport for Minors (below 18 Rs 600/-
years of Age) of 5 years validity or till
the minor attains the age of 18 which
ever is earlier.
4 Duplicate Passport (36 pages) in lieu of Rs. 2500/-
lost, damaged or stolen passport
5 Duplicate Passport (60 pages) in lieu of Rs. 3000/-
lost, damaged or stolen passport
6 Police Clearance Certificate/ Rs.300/-
