them to get the benefits they have earned and deserve
● We are an organization that serves our Active Duty Military, Reservists,
and Retirees.
● We specialize in attaining additional Military entitlements, of which
members may be unaware.
● We have been assisting the Military, in this capacity,
since 2010 in one form or another.

3. Program Overview
All Active Duty Military, Reservists and Retirees who are currently in enrolled
in TRICARE Health Plans are entitled to a
Health & Wellness Pack Benefit.
You can receive up to three benefits:
1.Pain-Free Relief Gel – an alternative to
oral narcotics and injectable steroids, for
joint and muscle pain
2.Skin Health and Wound Care Treatment – scar relief, with or without pain relief
3.A Daily Metabolic Supplement – help with overall wellness, with
a mixture of supplements designed specifically for the needs
of the TRICARE member.
This is at Little to No cost to all
TRICARE Members,
including spouses and dependents.

Scar Relief – treatment contains a mild steroid, designed to relieve redness
and discoloration around the wound and improve blood flow
Scar Relief with Pain Relief – works in similar manner, with light anesthetic

6. Benefit Overview: Metabolic Supplements
Alpha-Lipoic Acid. Powerful metabolic agent that has
been shown to help the remodel of collagen in improve the
look of skin. It is an antioxidant which has the potential to
diminish oxidative stress, improve the underlying
pathophysiology of neuropathy, and reduce pain.
Methylcobalamin. Is a active form of Vitamin B12 that
improves nerve regeneration and functional recovery. These

Resveratrol. Is a polyphenol, a natural chemical compound

Pyridoxal 5 Phosphate. An active form of Vitamin B6,
which is a natural coenzyme for fighting nerve pain. It
protects living lipids and proteins against glycation reactions
and aids in the proper function of sugars, fats, and proteins
in the body.

Complete and submit the patient Intake form to Creative Capital
Partners.
Step 2:
You will receive a Welcome call from our processing
department prior to your first entitlement shipment,
to verify that you have requested to be enrolled and to
verify your TRICARE member number that you provide us.
Step 3:
Your benefits will be shipped to the address you provided to us.
