A procedure used to safeguard employee’s from the
unexpected startup of machinery and equipment, or the
release of hazardous energy during service activities.

Incident Reporting
OSHA’s Form 300
An injury or illness is recordable if it results in
any of the following:
• Death
• Days away from work
• Restricted work or transfer to another
job
• Medical treatment beyond first aid
• Loss of consciousness
• A significant injury or illness diagnosed
by a physician or other licensed health
care professional
OSHA’s Form 301
This form provides additional details about
each injury or illness listed on OSHA Form
300, such as:
• Employee’s name, address, age,
gender, occupation, and department
• Place of accident and how it occurred
• Description of the injury or illness and
the parts of the body affected
