Surviving Out-Of-Hospital Cardiac Arrest: Chance or System? Basic interventions (bystander-CPR, early defibrillation) improve survival (even long term and neuro intact) but often we focus on advanced interventions which have no or weak impact on survival, particularly in the absence of basic interventions. Numerous app to alert first responders (trained and untrained laypersons, firefighters, police officers, and off-duty healthcare professionals) who are near a suspected OHCA were developed. Every European country is highly encouraged to implement such technologies in order to: - Improve the rate of bystander-initiated cardiopulmonary resuscitation (CPR). - Reduce the time to first compression and shock delivery. - Improve survival with good neurological recovery. Improving the first links of the chain of survival is the initial go-to strategy if you want to make a difference, everything else comes later and is extremely useful later. Durante la pandemia, gli arresti cardiaci extra-ospedalieri sono aumentati in modo importante in molte parti del mondo e hanno avuto esiti a breve termine peggiori rispetto a quelli avvenuti in un periodo precedente, suggerendo effetti diretti dell’infezione da SARS-CoV-2 ed effetti indiretti dal sovraccarico dei sistemi sanitari e l’accesso alterato alle cure (paura di recarsi in ospedale, sospensione delle visite). La pandemia COVID-19 è responsabile non solo dei decessi tra individui infetti, ma anche dell’eccesso di numero di OHCA associato minore probabilità di ROSC e sopravvivenza alla dimissione ospedaliera. Sebbene parte di questi risultati possa essere una conseguenza diretta del COVID-19, il lockdown, il distanziamento sociale e la riorganizzazione dei sistemi sanitari possono aver contribuito aumentando il tempo di arrivo dei soccorsi, gli OHCA che si verificano a casa non testimoniati e riducendo la RCP da parte degli astanti.

Sopravvivere all'arresto cardiaco extraospedaliero non dovrebbe essere questione di fortuna ma di sistema. Abbiamo forti evidenze di come interventi “basic” (massaggio cardiaco e defibrillazione precoce da parte di astanti, istruzioni prearrivo, allertamento di first responders, utilizzo di feedback devices) migliorino la sopravvivenza (anche a lungo termine e neurologicamente ottimale) e il numero di casi in cui l'astante esegue il massaggio cardiaco. Molto spesso ci si concentra però in interventi “advanced” (LUCAS, intraossea, ecmo, intubazione, farmaci, ...) che non hanno o hanno debolmente un impatto sulla sopravvivenza, in particolare in assenza degli interventi “basic”. Ad esempio nessun massaggiatore meccanico raddoppierà mai la sopravvivenza in assenza di un cittadino qualunque e il suo massaggio in quanto il suo ruolo principale è quello di “bridge” verso la terapia definitiva che porta a buoni risultati in centri con elevata esperienza nella gestione dell’arresto cardiaco. Migliorare i primi 2 link della catena della sopravvivenza è la strategia iniziale da seguire se si vuole fare la differenza, tutto il resto viene dopo ed è estremamente utile dopo.

Pediatric Weight Estimation With Augmented Reality Presented at 30° SMART Congress in Milan 8-10 May 2019 PediTape app is very simple and easy to use. After launching the app, the smartphone camera is immediately activated and displayed in full-screen with a yellow marker in the centre of the screen. The augmented reality software starts to track a correspondence between the real-world space and the virtual space: this is the essential requirement for any AR experience, and it’s called world tracking. World tracking also analyses and understands the contents of a real-world space using hit-testing methods detecting flat surfaces and main objects in the scene image. All these steps are needed in order to display the measuring tape together with a live camera image so that the user experiences augmented reality with the illusion that the virtual tape is part of the real world. After the completion of this process the app is ready to measure the length of the child by determining the two extremities points, head and foot. To start measuring child height you have to point the marker either to the head or to the foot of the child (starting point) and tap the marker by keeping the smartphone camera steady. After this action a virtual tape anchored to the starting point is displayed in the screen. From now, moving the smartphone around the child will increase the length of the coloured tape like a self-retracting metal tape measure. The next step is to tap while the marker is over the foot (or head if previously started from foot) defining the ending point.

Exploration: Despite its extreme conditions, the Mariana Trench has been the subject of numerous scientific expeditions and explorations. One of the most famous explorations was the dive to the Challenger Deep by Swiss scientist Jacques Piccard and U.S. Navy Lieutenant Don Walsh in 1960. More recently, in 2012, filmmaker James Cameron made a solo dive to the bottom of the Challenger Deep in the Deepsea Challenger submersible. Biological Discoveries: Despite the harsh conditions, the Mariana Trench is home to a surprising variety of life forms, including unique species of deep-sea fish, crustaceans, and microbial life.
