6. Siêu âm tuyến giáp bình thường
• Trên mặt cắt ngang, từ nông vào sâu gồm:
- Lớp da + tổ chức dưới da: tăng hồi âm và mỏng
- Lớp cơ : giảm hồi âm và dày hơn
- Nhu mô tuyến giáp : tăng hồi âm hơn lớp nông
• Bao giáp = đường tăng âm mỏng. Nhu mô có thể có các nang keo 2-
3mm không hồi âm, nốt vôi hóa hay dải xơ.
8. Kích thước tuyến giáp
• Chiều rộng: a < 2cm
• Chiều dài: b < 5cm
• Chiều dày trước – sau: c < 2cm
• Thể tích tuyến = a x b x c x π/6 < 10cm3
• Eo tuyến dày < 0.5cm
Chiều dày trước – sau là chính xác nhất,
khi > 2cm tức là tăng kích thước

11. Vô sản tuyến giáp
• Siêu âm: Mô hồi âm bất thường ở vị trí tuyến giáp và không có tín hiệu dòng
chảy bình thường trên Doppler. Không có bằng chứng về tuyến giáp lạc chỗ
• Xạ hình: cho thấy mô tuyến giáp không có chức năng.

13. Tuyến giáp lạc chỗ
• Trong thời kì bào thai, tuyến giáp di chuyển xuống vị trí bình thường
qua ống giáp móng (thyroglossal duct). Nếu bất thường trong quá
trình đi xuống lạc chỗ
• Các vị trí: trong, dưới lưỡi; trước hầu; trong khí quản; trên xương ức.
• Đánh giá sâu sử dụng CT/ MRI hoặc xạ hình
14. Nang giáp móng
• Do ống giáp lưỡi không teo đi sau
khi sinh
• Là nang bất thường vùng cổ hay
gặp nhất
• Có thể nhiễm trùng, hoại tử hoặc
thoái hóa thành ung thư

Bệnh nhân nam 35T với 2 tuần sưng vùng giữa cổ

25. Bờ
• Vòng Halo ngoại giảm âm ngoại vi có ý nghĩa nốt có tính chất lành tính
cao gấp 12 lần so với tính ác tính (92%)
• Vòng Halo không hoàn toàn thì cao hơn 4 lần (80%)

• Vôi hóa bờ ngoại vi / các vùng vôi hóa thô và lớn thường lành tính

First, about the anatomy
The thyroid gland is located anterior and lateral to trachea below the level of thyroid cartilage and above the sternal notch
It is composed of:
Right + Left lobes
Isthmus
Sometimes Pyramidal lobe may present (10-40%)

Normal thyroid parenchyma has homogenous medium to high level echogenicity and bounded by a thin hyperechoic line (the thyroid capsule)

NORMAL DIMENSIONS OF THYROID LOBES
- The normal thyroid gland is 2 cm or less in both the transverse dimension and depth, and is 4.5–5.5 cm in length & 5 mm for the isthmus.
The length is measured from the two ends of the lobe in the longitudinal view.
A-P diameter is most precise, when > 2cm means enlarged gland

THYROID AGENESIS
- Ultrasound : Abnormal echogenic tissue in the expected location of the thyroid, without any normal flow on color Doppler imaging. There is no evidence of ectopic thyroid tissue.
- Scintigraphy demonstrates no functioning thyroid tissue.

ECTOPIC THYROID
Normally, the thyroid gland descends to its typical location anterior to the cervical trachea via the thyroglossal duct.
Anomalies of descent can lead to ectopic thyroid.
This duct normally goes away once the thyroid reaches its final position in the neck. If Some part of the duct remains, This leaves a pocket called a cyst.

For the solid/cystic ratio
Nodules with large cystic components are usually benign nodules that have undergone cystic degeneration or hemorrhage , however about 20-30% of papillary carcinomas also have a cystic component, indicating that not all cystic thyroid nodules are benign.
One highly specific sign of a benignity of a thyroid nodule is the presence of comet tail sign, signifying a benign colloid nodule.
This sign is never encountered in a malignant lesion
Echogenecity patterns:
Hyperechoic (> thyroid) malignant in 4%
Isoechoic (= thyroid) malignant in 26%
Hypoechoic (< strap muscles) malignant in 63%
Margins
- Peripheral halo of decreased echogenecity is seen around the nodule.
- A complete halo is 12 times more likely to indicate benign lesion
- An incomplete halo is 4 times more likely benign than malignant
SHAPE
Taller than wide 93% specific for malignant lesion
Calcification
- Peripheral rim calcification and large areas of coarse shadowing calcifications are more frequently seen in benign nodules’
- Microcalcifications ≤ 1mm are more frequently seen in malignant nodules
Color flow imaging
There are three general patterns of vascular distribution of the thyroid nodule:
Type 1 : No flow signal within the nodule.
Type 2 : exclusive perinodular arterial flow signal
Type 3 : intranodular flow with/ without perinodular flow
“type 1 and 2” are more common with benign nodules
type is generally associated with malignant nodules.
This feature is more useful in selecting a nodule for FNA in multinodular goitre
Although none of the ultrasound features above is BY ITSELF specific for malignancy, BUT when it used IN COMBINATION they are very useful in differentiating malignant from a benign nodule
Examination of adjacent structures
(1)- common carotid artery and internal jugular vein : the presence of thrombus within CCA or IJV in association with a thyroid nodule is a clue to the malignant nature of the nodule
In this picture we see that the lesion invades adjacent structures (here is strap muscle)

For lympho node, we have to distinguish characteristics of benign and malignant lesions based on these followings.
Benign lymph node
- This lymph node appears benign because it is flat with a short/long axis ratio < 0.5
- Power Doppler of the previous lymph node shows vasculariation of the hilum, which contains small arterioles. Note there is no vascularization seen in the periphery of the node.
Malignant lymph node.
This lymph node is slightly more rounded, with a short/long axis ratio > 0.5 in the transverse view. Note the absence of a hilar line, which makes this node suspicious.
FNA is needed to confirm malignancy.

The calcification at 3:00 o’clock indicates it is malignant, but UG FNA is necessary before surgery.

Knowing this is necessary when we type the ultrasound result as it will help the reader to indentify the location of the lymph node when doing FNA or biopsy.

TIRADS is necessary when we assess a lesion and should be accompanied when typing ultrasound result as it helps the reader to make out prognosis and further indication
According to TIRADS, we have 5 dangerous signs on ultrasound. One of them is about elastography so here we only care about the other 4 signs, which are: XXX

Multinodular lesions
MULTINODULAR THYROID
It is generally believed that malignancy is common in a solitary nodule and that multinodularity is usually associated with benign disease, HOWEVER 10 – 20 % of papillary carcinoma may be multicentric
For diffuse diseases of the thyroid, we mention about followings:
Grave’s disease
Hashimoto thyroiditis
De Quarvan thyroiditis
- Goiter is an abnormal enlargement of thyroid gland
On the picture is an example of multinodular goiter.
+ Enlargement of thyroid lobes and isthmus
+ multiple hyperechoic solid nodules with uniform thin halo (arrows).
+ Mixed solid and cystic thyroid nodule in the left lobe.
+ On Doppler show a well-defined isoechoic thyroid nodule with thin complete hypoechoic halo, intranodular cystic/colloid space and peripheral vascularity, findings indicative of a hyperplasic nodule.
Most common cause of hyperthyroidism
includes symptoms of:
Hyperthyroidism + Goiter + thymoma
Signs on ultrasound:
+ Diffuse enlarged gland
+ Heterogenous echogenicity with hypoechogenic areas
+ Hypervascularity on Color doppler
Three stages:
Acute: enlarged in size and increased vascularity.
Chronic: enlarged gland with multiple linear bright echoes throughout the hypoechoic parenchyma as well as multiple small hypoechoic nodules.
Atrophic; end stage : small atrophic gland. avascular with heterogenous echoes.
Enlargement of the thyroid is due to lymphocytic infiltration and fibrosis rather than tissue hypertrophy
Chronic thyroiditis is likely the most common cause of hypothyroidism and goiter
The inflammation does not involve the entire gland but infiltrates the gland in a pattern of hypoechoic and hypervascular areas
