35. Chống chỉ định
Máy tạo nhịp hoặc khử rung.

Đang hoặc có tiền căn ung thư da, hoặc đang bị 1 loại ung thư khác, hoặc nốt ruồi tiền ác
tính.
Tiền căn bị ung thư. *
Các bệnh lý nặng như rối loạn tim mạch.
Đang mang thai và cho con bú.
Bệnh lý rối loạn miễn dịch như HIV, hoặc đang sử dụng thuốc điều trị rối loạn MD. *
Bệnh lý có thể bị kích thích bởi ánh sáng có bước sóng đang sử dụng, như tiền sử bị Lupus
ban đỏ hệ thống, Porphyria, và động kinh. *
BN với tiền căn bệnh lý bị kích thích bởi nhiệt như Herpes Simplex ở vùng điều trị, có thể
được điều trị khi có sự theo dõi của BS điều trị.

có thể điều trị dưới sự giám sát và chịu trách nhiệm của bác sĩ điều trị , tuy nhiên không khuyến khích. Trong 1
số trường hợp điều trị 1 vùng nhỏ trước sau đó vài ngày mới quyết định nếu BN đáp ứng được và không có
thêm tác dụng phụ.
36. Chống chỉ định
Rối loạn nội tiết tố không kiểm soát được như tiểu đường, or PCO for hair
removal.
Bất kỳ bệnh lý hiện tại nào tại vùng da điều trị như đau nhức, vẩy nến, chàm
và phát ban.
Tiền sử bệnh lý da , sẹo lồi, lành thương chậm cũng như da khô và giòn.
Tiền sử chảy máu không đông, hoặc đang sử dụng thuốc kháng đông.
Sử dụng thuốc, thảo dược, thức ăn mà làm gia tăng nhạy cảm ánh sáng
của da tại bước sóng đang sử dụng như Isotretinoin (Accutane) trong vòng
6 tháng, Tetracyclines, or St. John's Wort trong vòng 2 tuần. *
37. Chống chỉ định
Điều trị tái tạo da mặt bằng laser và lột sâu bằng hóa chất trong vòng ít
nhất 3 tháng, nếu mặt được điều trị.
Bất kỳ phẫu thuật nào ở vùng điều trị trong vòng 3 tháng hoặc trước khi
hoàn tất quá trình lành thương.
Nhổ lông, waxing trong vòng ít nhất 6 tuần trước khi triệt lông.
Điều trị trên vùng xăm hoặc trang điểm vĩnh viễn.
Tắm nắng hoặc kem làm nâu da trong vòng ít nhất 2 tuần.
Bất kỳ ý kiến chủ quan nào của người thực hiện cho rằng việc điều trị là
không an toàn cho BN.
38. Mong muốn thực tiễn
Nhiều lần điều trị và thỉnh thoảng lập lại
Chống lão hóa có thể gây xuất hiện mạch máu mới, sắc tố và sang chấn cấu
trúc
Chu kỳ mọc lông. Nang lông mới có thể bị kích hoạt từ trạng thái nghỉ ngơi
Cần thời gian mới đạt hiệu quả!

39. Tác dụng phụ (hiếm)
Khó chịu
Sưng tấy/đỏ da
Bỏng có hoặc không có bóng nước
ảnh hưởng đến da bình thường xung quanh
Thay dổi sắc tố:
(mất hoặc đậm màu sắc tố)
Sẹo
hầu hết những tác dụng phụ này là tạm thời và
điều trị được và thường xảy ra ở người có
màu da sậm
Không nên tiếp tục điều trị khi chưa giải
quyết hết tác dụng phụ!
40. Điều trị tác dụng phụ
Phản ứng thái quá của da
Ngay lập tức chườm lạnh lập đi lập lại bằng gạc lạnh
Làm dịu da, như Aloe Vera
Phồng rợp đóng vảy
bôi kem chóng bỏng, steroid, hoặc kháng sinh
Sậm màu da
Tự khỏi. Có thể mờ dần trong vòng 2-6 tháng
Dấu hiệu tạm thời có thể xảy ra khi điều trị bằng đầu SR/SRA
Mất sắc tố da
Nên để tự khỏi ( 6 - 24 tháng, hiếm khi vĩnh viễn)

Sẹo lồi: chích steroids
Sẹo lõm có thể sử dụng đầu Matrix-RF hoặc chích chất làm đầy
Sẹo đỏ có thể điều trị bằng SR or LV/LVA
41. Chuẩn bị trước điều trị
Chụp hình
Làm sạch vùng điều trị
Cạo hoặc che chắn lông
Có thể gây tê tai chỗ ngoài da: EMLA, L.M.x.4, Ane Stop, Lidocaine
4% trước 0.5-1 giờ (hiếm).
Mang kính bảo vệ mắt cho tất cả mọi người trong phòng điều trị
Kiểm tra rằng BN đã ký tờ cam kết
42. Tờ cam kết (ICF)
ICF nên được ký ngay trước khi điều trị
ICF nên được chỉnh sửa ở mỗi lần điều trị tiếp theo
bằng các câu hỏi:
Lần cuối phơi nắng là khi nào?
Có thay đổi gì trong bệnh sử không (mang thai/tiểu đường)?
Có đang uống loại thuốc nào không?
43. Quy trình điều trị
Chẩn đoán loại da:
Loại da tùy vào chỉ định
Làm Test Spots – để tránh tác dụng phụ
Luôn luôn làm test trước khi điều trị toàn vùng
Bắt đầu từ mức năng lượng thấp và tăng lên từng nấc
Đánh giá phản ứng của da:
• Da sáng chờ vài phút
• Da sậm chờ 24-48 giờ
Kỹ thuật phát xung – bôi gel dẫn lên da vùng điều trị và đặt
đầu điều trị vuông góc bề mặt da và ấn nhẹ (ngoại trừ mạch
máu nông) và tiếp xúc toàn bộ
Chồng lắp – 10- 50%, tùy vào đầu điều trị
44. Thông số điều trị chung
Theo hướng dẫn trong sách hướng dẫn điều trị (QRG’s)
Mức năng lượng:
Mức thấp hơn cho da sậm

46. Chăm sóc sau điều trị
Có thể làm lạnh sau điều trị với túi chườm lạnh (không đông) , làm lạnh
bằng khí lạnh của máy SynerCool,…
Bất cứ phản ứng phụ nào phải báo ngay cho người điều trị
Giộp hoặc loét da có thể điều trị bằng kháng sinh bôi hoặc kem chống bỏng
Tránh những thủ thuật gây chấn thương da trong 2 ngày đầu sau điều trị
như xông hơi, massage, cào gãi…
Nếu vảy xuất hiện sau phồng giộp, nên giữ mềm bằng kem nhờn.
Kem chống nắng chỉ số cao (tối thiểu 30 SPF) nên sử dụng tối thiểu 1 tháng
sau điều trị
47. Kết hợp điều trị với các phương pháp khác

The history of optical-based systems for aesthetic and medical use started with Lasers in 1970.
In 1995, Dr. Shimon Ekhouse introduced the Intense Pulsed Light (IPL), which enabled a broader spectrum of clinical applications in various depths of the skin.
In 2000 Dr. Shimon Ekhouse refined the IPL and Laser technology by introducing the 3rd generation technology, namely elos. Elos is based on the combination of IPL or Laser with Radio-Frequency (RF).
RF has a dual definition as particles (photons) and as waves; the photons carry energy and travel as waves.
The wavelength is defined as the distance between 2 peaks of the wave.
Short wavelengths are more frequent in a time unit (sec) and their photons can carry a higher proportion of output energy than long wavelengths. The penetration depth of short wavelengths into tissue is smaller than that with long wavelengths.
The electromagnetic spectrum is the range of photonic energy sources. IPL is a broad spectrum, usually in the visible and near infra-red range.
The total spectrum of the electromagnetic radiation spreads far beyond the visible light (390-760 nm). In the visible light range, changes in wavelength are perceived by the human eye as changes in color.
The shorter wavelengths include the cosmic rays, x rays and ultra violet, which are very powerful and may cause chromosomal DNA break down, mutations and cancer - NOT PRESENT in Syneron systems.
The longer wavelengths such as microwave radiation which is highly absorbed in water and may cause tissue thermal damage are also NOT PRESENT in Syneron systems.
The electromagnetic spectrum is the range of photonic energy sources.
The difference in energy source relates to the wavelength of that type of energy.
The visible range of EM means human eye can see these wavelengths of light.
Shorter than violet (approx 400nm – ultraviolet) and longer than red (approx 700nm – infrared) are invisible to the human eye.
Certain wavelengths can be used for specific target destruction based on absorption strength of light by the target.
Example, hemoglobin (blood) related targets absorb strongly between 480nm-630nm. Melanin or pigment targets absorb strongly between 694nm-755nm. Water absorbs wavelengths between 2000nm-10600nm. Microwaves, TV, and radiowaves are very long wavelength energies.
The mechanism of target destruction by Light or Laser was described by Rox Anderson in the 80ths. The theory is based on the absorption of the optical energy by relevant chromophores, the transformation of absorbed light/laser into heat, and the selective destruction of the target, with no thermal damage to the surrounding tissue.
This slide demonstrates the absorption capacity of different materials at various wavelengths. It explains the wavelengths choice for the various applicators.
It can be seen that water absorption between 0.5 and 1.0 um is low and can protect the tissue from boiling. CO2 and Er:YAG, which are highly absorbed in water are indeed used to ablate and evaporate the epidermis in skin resurfacing procedures.
Melanin absorption of light is reduced in longer wavelengths. A compromise should be done between maximal absorption in pigmented lesions or hair and minimal absorption in the epidermis. The darker the epidermis, the more careful the user should be and use lower optical energy.
Hemoglobin is absorbing well in short wavelengths (500-600 nm). When deeper veins, like blue leg veins, are the target, and a longer wavelength has to be used to reach it, the low absorption has to be compensated by using high energy output.
One of the biggest problems of treatments based on optical energy is the high incidence of burns and the consequent adverse events such as pigmentation changes of the skin and scars.
Since there is a high percentage of optical energy loss by the treated skin, in order to be effective, a high energy output is necessary. This high energy when penetrating through the epidermis can cause burns.
Reaching the target would be an easy task if the epidermis and dermis where transparent. But, since they are not transparent, light penetration through them can be compared to light penetrating through a curtain on a window. The amount of light transmitted would be determined by the color and the texture of the curtain. The darker the curtain, the more light will be absorbed and the less light will penetrate inside the room. As with the curtain, the darker the skin the more light it will absorb by the melanin in the epidermis and the epidermal/dermal junction, and less photons will reach the target. The photons absorbed in our natural curtain, the skin, will produce heat which may harm the epidermis.
When aiming for photothermolysis, one is not only attempting to protect the epidermis, but is also interested in minimizing the epidermal melanin absorption as a competition, so that sufficient light energy can reach the target.
The high burn incidence called for the use of lower optical energy, but in order to be effective, an additional energy source which is not absorbed my melanin (color blind) was needed. Syneron decided to use the electrical current of radio frequency (RF) as a compensating energy source which finally would heat the target and help destroy it.
There are 2 forms of RF:
Radiation RF which is radiating from a distance with no contact with the skin.
Conductive RF whereby the electrodes have to be in contact with the skin.
There are 2 types of conductive RF:
Monopolar – one electrode is touching the skin and the other one is held by the hand or tied to the back. The electrical current then flows through the body.
Bipolar – both electrodes are touching the skin and the area of current flow is limited.
Radiation and Monopolar RF penetration depth into the body is not controlled, neither is the amount of electrical energy that is passing in various body areas.
The bipolar RF has a controlled penetration depth, as well as energy input.
RF is very safe and controllable energy used for many years in medicine , although not in combination with optical energy.
The frequency used is suitable for sufficient penetration depth and for safety in the skin.
Its heating properties can be measured by recording voltage and resistance.
There is no energy loss so the amount of energy in the skin is known.
The penetration depth is roughly half the distance between the electrodes. Thus superficial lesions may be treated by small distant electrodes (~4 mm) and deeper ones, such as fat, by further apart electrodes (10-15 mm).
Since there is no selective effect on melanin, there is no over-heating in dark skin.
At the same time targets with little optical absorption may be heated by RF.
The penetration depth is depending not only on the distance between the electrodes, but also on the tissue temperature, which in turn affects the tissue electrical conductivity.
By cooling the epidermis, its electrical conductivity drops and RF is pushed deeper.
By pre-heating the target, its electrical conductivity rises and it will attract most of the RF.
Presence of pigment that absorbs the specific wavelength in the target only is a must to get the heat selectively in the target.
RF, is independent of pigment presence or absence. It will simply flows where there is the easiest way to flow: the shortest way or where there is least electrical resistance.
Having 2 separate mechanisms of selectivity, is making the overall treatment more selective than each of the technologies alone.
The parameters that affect the treatment efficacy, safety and selectivity are:
Target pre-heating by light or laser
Epidermal cooling
Synergistic effect
Electrical current will always travel the shortest distance via the path of least resistance. To protect the epidermis and push current deeper, the surface can be cooled and made more resistant to current flow. The current then prefers to flow deeper. This can be clearly seen by the white spot (coagulated tissue) shown in these images of treated slice of meat.
Synergism is shown by the higher temperature and the deeper penetration of the combined energy sources in comparison to each of them alone.
The lowest optical energy in combination with RF yields a higher and deeper temperature than x2 or x3 higher IPL alone.
There is an inverse relation between the temperature rise and the electrical impedance (resistance) drop in the dermis.
The drop of skin impedance, as a result of heating is measured every 10 ms. It is only displayed once, at the end of the pulse.
For safety reasons, the impedance drop is restricted to 30%, which in turn restricts the temperature rise. If a lower dermal temperature is required (e.g. in dark skin), the impedance drop should be lower than 30%.

Proper patient selection includes the exclusion of medically contraindicated patients, as well as the exclusion of patients with excessive lesions.
Realistic expectation is essential to have satisfied patients and to avoid legal acts.
Parameters, techniques, and protocols have to be taught to new users according to Syneron education plan. There is a learning curve of a few months. Only later on, they may learn from luminaries, or develop other treatment processes of their own.
There is no one modality or one application that may give a complete solution. Combination treatment with a few of Syneron systems or with other modalities may give more comprehensive results.
The end-point of treatments should be efficacy, but safety comes first. More sessions and longer treatment schedule using safe parameters is better than taking a risk.
Epidermal or junctional pigmented lesions could be treated by IPL+RF. Deeper dermal pigments lesions are not indicated (should be treated with Q-switched lasers).
Superficial dermal vascular lesions (up to 0.5 mm diameter and depth) are treated by IPL+RF, as the deeper dermal blood vessels can be treated by Laser+RF.
Acne in the sabeceous glands that open into the hair follicles is treated by IPL+RF.
Dermal collagen fibers may be treated for tightening and wrinkle and scar improvement by both IPL and Laser+ RF.
Hair follicles that are epidermal invaginations in the dermis or hypodermis can be treated by both IPL and Laser+RF.
The upper part of the hypodermis is the target treatment of the VelaSmooth/VelaShape combined technologies.
The epidermis is comprised of several cell layers. It’s depth is 70-100 microns. Most of the cells are keratin-producing cells (keratinocytes) which divide and multiply at the basal layer about once a month, and as they are pushed towards the surface they produce more and more keratin. The surface layer is devoid of living organelles and are dead firm cells that serve for protection (e.g. keeping body temperature, and humidity and preventing penetration of foreign bodies).
In the basal layer there are some large melanin-producing cells (melanocytes) which transfer the melanin to the neighboring keratinocytes. The amount of melanin (produced by the same number of melanocytes) is an ethnic characteristic. The response to exposure to light is producing more melanin and spreading it in the epidermis to form a curtain.
There are no blood vessels in the epidermis. it is nourished by diffusion from the dermal vessell.
The pigmented lesion, which are pathological clusters of melanin and melanin-containing cells, are formed in the epidermis, but may sink into the dermis.
The dermis depth varies according to body location and is 2-5 mm. The main cells in the dermis are fibroblasts which produce collagen and elastin fibers and secrete them to give the skin its rigidity and elasticity. The inter-cellular hydrated materials (such as hyaluronic acid) are also produced and secreted by the fibroblasts.
The dermis is rich in blood vessels, lymph vessels, nerves and immune system cells.
Skin type may be characterized by the 2 different scales. Although every individual has got one specific type, there may be variations in color in different body locations. Thus areas which are exposed more to sun, such as face, will be darker that hidden areas. There are also different natural coloration, such as the inner and outer bikini line. The color should be taken into consideration when treating a specific area.
The Fitzpatrick scale is based on the response of the skin to 30 min exposure to sun without sun block – tan or burn.
When in doubt, always classify as darker for safety when starting treatment.
The ethnic group and the usual relevant hair and eyes color, may help in skin classification.
The first group of contraindications is due to RF hazards. When there are superficial metal implants in the teeth, it is possible to space them away from the applicator tip by pushing the surrounding area away (by gloved hands, spatula, or condensed cotton wool). Metal implants in bones are usually deep enough, however with any discomfort, treatment on this area should be terminated.
The second group of contraindications do not imply any known clinical hazard, but are due to liability issues.
Some diseases are contraindicated, but when marked with star, they may be treated under the physician’s discretion and responsibility, although not recommended by the company.
Diabetic patients are ~10% of the population. If they are well balanced , there is no problem in treating them. However, unstable conditions should be contraindicated, as the hazard of hemorrhage of the fragile capillaries is high. Poly Cystic Ovary, if not treated, will render hair removal not effective.
Genetic tendency to keloids is abundant among Asians. Any skin damage may develop into a keloid - do not treat.
Anticoagulants are contraindicated due to hemorrhage hazard. however, when only mild low dose is used, such as 75 mg aspirin, the physician may try cautiously (small area and mild parameters).
Photosensitive drugs are contra-indicated according to the wavelength used. Herbs like St. John’s Wort antidepressant may also be photosensitive. The patients can be treated if medication is stopped for at least 2 weeks before treatment with a doctor consent. If not stopped, It is possible to treat with caution following a doctor consent.
Needle epilation often results in hair shaft shedding. Since the hair shaft serves as an antenna to capture the light, it is not recommended to treat. Needle epilation also interferes with the hair cycle and it is difficult to schedule the treatments.
Tanned skin is more risky to treat than originally dark skin. If the tan is not extensive, elos has an advantage over light/laser systems, as low optical energy may be used for safety and compensated by RF.
There are many clinical conditions that are not mentioned in this long list of contraindications. In any case of doubt, it is suggested to get a consent from the patient’s personal doctor.
The long contraindication list may look alarming, but if the answer to 2 questions:”Do you suffer and have you suffered in the past from any disease?” and “Do you take any medication?” is “No”, the list becomes very short.
Realistic expectations are extremely important when performing elective aesthetic procedures, which are usually quite costly.
Usually, when there is more hazard for burns, such as in dark skin, or on dense lesions, it calls for milder parameters which in turn may require more treatment sessions and possibly a smaller percent clearance.
Although the possible adverse events are rare, patients must sign on the consent forms that they are aware of them.
The rareness of the side effects with Syneron systems is the reason why insurance companies in USA are charging the doctors using these systems a low premium for medical liability.
Photographs of “Before” are essential. “After” photos can be taken later. One photo is worth a 100 words in case of claiming no results.
The treatment area should be cleaned off make-up, creams, deodorants etc. with a non-alcoholic preparation to avoid skin dehydration. Alternatively, the treatment gel may be used.
Hairy areas (men’s beard) which are not the treatment target, treated for applications other than hair removal such as SR/SRA, should be avoided. Otherwise, there is some chance that unwanted epilation will occur.
EMLA is vaso-constrictive and should not be used when superficial vessels are treated. It is always better to avoid topical anesthetics, as the real feedback from the patient helps to avoid side effects. However, on large treatment areas such as back or legs it can be used, after the parameters were determined without it. Topical anesthesia may be used by the discretion of the practitioner and on his own responsibility, and on limited body areas only, ascertaining that the patient is not allergic to the components.
SynerCool or any other cooling measure is a better alternative as it can reach deeper, such as in hair removal or leg vein treatment.
Before each treatment make sure that there is no change in the patient medical history as documented in the ICF. In case of a change, add a note to the ICF and make the patient sign again.
If possible, test spots should be performed on non-conspicuous areas that contain your target.
All skin types may be treated with ST, AC, DS/DSL, and VelaSmooth applicators.
Pigmented lesions and wrinkles may be treated up to skin type V.
Vascular lesions may be treated up to skin type IV, as there is hardly any difference in color between the darker skin and the lesion.
Dark skin should be treated cautiously by experienced users only. A test spot can be done in the consultation session to avoid additional session.
If the electrodes are not perpendicular to the skin, the contact area of the electrodes is narrower, and a higher energy density may cause a burn.
The overlap is needed along the electrodes only, as the light/laser is not emitted in the site of the electrodes.
Most cases of burn result from excessive optical energy.
RF, unless having a high energy density (Fluence) on thin skin over bone, or if only part of the electrodes are touching the skin, is usually safe.
Tanning after treatment may cause hyperpigmentation, which is the natural defense mechanism of the skin in exposure to sun. As some of the melanin has been destroyed during the treatment, over production of melanin may sometimes occur.
Combination treatment on the same area may be done in the same session, if skin response and patient tolerance allow.
Usually SR, followed by ST, then followed by WRA/Matrix IR may be done in the same session. Same applies to VelaSmooth and ST.
Usually leg veins, acne and hair removal will involve different applicators in different sessions.
The described treatments are usually done on the face , but may be applied to any other body area. In combination, usually the first 3 may be performed sequentially in the same session, if skin response allows.
Usually, additional facial treatments, such as for acne (AC), for hair removal (DS, DSL) or for deeper veins (LV/LVA), are performed in subsequent sessions, when the protocol for skin rejuvenation and collagen remodeling is terminated.
Following elos treatment, the first response is collagen fibers shrinkage due to the heating effect. Further heating destroys the collagen fibers and stimulates the immune system cells to reach the damaged area. Along with, fluids that come out of the blood vessels to the tissue create edema and an immediate transient tighter look (together with the shrunk collagen). These fluids contain factors that stimulate the production of new collagen fibers by the dermal cells (fibroblasts), a process that may take 6-8 weeks.
One treatment session may not give sufficient amount of collagen, but following 2-3 sessions, the textural results may stay for a few months.
Stained histological sections show the spaced and disorganized old collagen fibers of aged skin. Upon elos treatment, the new collagen fibers appear more compact and organized.
The simultaneous treatment of browns, reds, and texture is emphasized.
It should be noted that the all the treated lesions are superficial: pigmented, vascular and textural.
Following the light absorption, the energy is converted into heat in a photo-thermal reaction and consequently two processes occur: Coagulation caused by denaturation of proteins (e.g. cooked eggs) and thermally-induced mechanical damage.
Thus, the blood is coagulated by the heat The endothelial cells (the inner lining of the vessel) are damaged. There is no complete perforation – no risk of hemorrhage. Some small blood vessels collapse and disappear (vasoconstriction), but they may reappear if sufficient damage is not done to the walls.
Melanin granules explode and destroy the cells that harbor them.
Erythema and edema are signs of cellular damage that recruit the immune system to eliminate the damaged cells, as well as blood clots or broken melanin particles.
The skin around should stay intact and undamaged!
Phagocytosis – the process of elimination of small damaged particles and debris “swallowed” by the white blood cells, which are recruited from the skin and out of the blood vessels to the damaged tissue area. The clearance process may take up to a few weeks and even months, depending on the lesion size. Large leg veins may take 6 month to be cleared away.
Skin type V and tanned skin should be treated cautiously by experienced users only.
Make sure that patients are warned about darkening of pigments and mini crust formation as positive results indications.
Non-facial skin is usually thinner and more sensitive. When using harsh techniques, such as skin resurfacing there is a great hazard of hyperpigmentation and scarring. Therefore for these areas, skin rejuvenation is the preferred solution.
Exact number of session depends on the preliminary condition and on the individual response.
Pigmented lesions and fine telangiectasia must be treated in Short pulse type in order to be effective. Deeper vascular lesions may require longer time to heat, so Long pulse is suitable.
Larger vessels require lower optical fluence than small ones, as they contain more hemoglobin and absorb better the optical fluence.
Dark pigments require lower optical fluence than light ones, as the higher concentration of melanin yields a better absorption of the optical fluence.
The darker the skin, the lower the optical fluence. As the targets become lighter and less dense in subsequent sessions, it is possible to try and increase the optical energy.
Usually, RF is maximal (25 J/cm3) on soft tissue and lower over bone (15 J/cm2).
Generally, pigmented lesions require lower optical fluence than vascular. This is due to the relative low concentration of hemoglobin in the blood, compared to melanin concentration in the lesions. In addition, hemoglobin has definite wavelength peaks of absorption, whereas melanin absorption is gradually decreased with increased wavelength.
Superficial vascular lesions which are treated with relative high optical fluence may heat up excessively, therefore it is helpful to pre and post cool. This cooling should not be excessive, as it may cause vasoconstriction, and sort cooling (1-2 sec) with the light-guide tip, and not with more drastic cooling, may help.
General skin response of slight erythema and/or edema is acceptable, but not essential.
Vascular lesions darken post treatment due to coagulation by the heat.
Erythema/edema are positive signs of immune system recruitment.
Vasoconstriction and no blood refilling after pressing the vein and releasing, should be tested after 5-10 minutes. If the blanched vessel reappear or if there is blood refilling, another pass may be attempted.
The pigmented lesions will darken after treatment due to aggregation of the smaller broken melanin particles outside the granules and/or the cells. Patients have to be warned that the pigments will look worse than originally for a few days, occasionally 1-2 weeks.
The mini crust is a part of the healing process of the heated tissue around the epidermal pigmented lesions.
The textural improvement of the papillary dermis is due to the contracture of the collagen fibers.

The SRA applicator (vs. SR) is a more aggressive applicator due to the shorter wavelength range and the shorter pulse duration. Therefore it is more suitable for resistant lesions (pigments and fine vessels) on light skin.
As with SR, the darker the skin, the lower the optical fluence, which is ~30% lower than the SR. Usually, RF is maximal (25 J/cm3) on soft tissue and lower over bone (15 J/cm3).
When SRA is applied to darker skin (IV-V), very low optical parameters should be used, and only cautiously by experienced users.
The patients must be warned that the immediate response will not look good for a few days. In most cases, make-up may be used to cover up the intense response. However, care should be taken when removing the make-up and it should be done very mildly, if at all, to avoid skin irritation.

This photo shows the use of SRA for the treatment of melasma. However, it is recommended to try SR first for a couple of sessions.
Before treating, always refer to a dermatologist to determine if the lesion is melasma and in what depth it lies. Only superficial (epidermal and junctional) melasma can be treated with elos. Melasma is an hormonally induced disease and may reoccur. When treated, the response may be: improvement, no change, or worsening. To avoid worsening, lower parameters than for lentigines should be used, for both optical fluence and RF. Optical fluence should be ~30% lower, and RF 15-20 J/cm3.
Melasma is best treated in combination with bleaching agents and peeling according to physician’s protocol.
The nasal vessels are usually very resistant. There is no need to wait for 3 sessions with SR, and SRA may be applied from the start.
Tightening is based on collagen shrinkage and denaturation all over the dermis – superficial and deep. In order for it to be homogeneous, the heating should not be focused, but rather spread all over. Since water and blood vessels are all over the dermis, they heat up by IR and RF and conduct the heat to the collagen fibers.
This thermogram demonstrates the safety to the epidermis that stays cold and the efficacy to the treated area that heats up. Since each pulse heats up the target in ~10oC only, several passes are needed to reach a temperature near 60oC which destroys the collagen fibers and induces new collagen formation.
The areas to be treated should not be too large, so that the heat will accumulate. At the same time, the area should not be too small (<10), as heat will accumulate too fast with a burn hazard.
Since the ST treatment is mild and safe, it is advised to do up to 50% overlap.
The vector zones that are treated are according to the individual needs.
Tightening is achieved when treating zones 2, 3, 4, 6, 7, 8.
Wrinkles are treated on zones 1, 5, preferably by WRA or Matrix IR applicators, if available.
Although collagen fibers are in all directions, the treatment directions are recommended in accordance with the patient’s perception of the tightening direction.
The protocol of SR/SRA is similar to the ST, so it is easy to do a combination treatment in the same session.
All skin types may be treated, usually with maximal RF. IR is fixed and low.
If not comfortable, the Cooling may be changed from Normal to Strong, mainly on bony areas. If it is still uncomfortable, the RF may be lowered, but not less than 90 J/cm3.
The number of passes may vary individually, until end-point is reached. To check the end-point of tightening and some edema, the patient should be seated up and the treated side compared to the untreated one.
The difference between the treated and untreated sides was sustained for 12 months.
Follow-up of one month after a full protocol of 5 sessions.
Follow-up of 1 year.
Photodynamic therapy (PDT) + hyperthermia destroy selectively the P.acnes bacteria.
In comedones (white heads and black heads), where there are very few bacteria, RF and red light heat and dry the sebum in the sebaceous glands, thus preventing the process of inflamed acne. Not only the bacteria are destroyed, but their micro-environment of excess sebum is also destroyed.
The textural improvement is due to collagen shrinkage by the RF.
The first pathological symptoms of acne are the microcomedones which are only histologically diagnosed under the microscope. May develop into non-inflammatory comedones or inflamed lesions.
Non-inflammatory acne
Comedones – Left: Whiteheads (closed) lesions, are hardly visible as they are white or skin color, slightly elevated but contain a lipid plug and if they do not go through spontaneous remission, they may develop into blackheads or inflamed papules.
Right: Blackheads (open) lesions, exposing dark central impaction of keratin and lipids, oxidized by exposure to air. They are flat or slightly elevated.

The main porphyrin produced by P.acnes in its membrane is coproporphyrin III. This porphyrin has a peak of absorption at 400-420. Competitors using red light or green light are not using the optimal wavelength for maximal absorption.
Photo-excitation starts a photodynamic cascade whereby the excited porphyrins transfer their excess energy to molecular triplet oxygen, so that a non-stable singlet oxygen is formed.
The cytotoxic singlet oxygen releases its excess energy by peroxidation of structures in the bacterial wall. Bacterial functions are impaired, resulting in bacterial death.
This reaction is selective and is confined to P.acnes and their inner membrane-bound porphyrins and is very short, so no singlet oxygen is available to destroy the human skin cells.
Various topical treatments or mechanical abrasion, may be combined according to physician discretion, to improve results.
SR/SRA is usually done on residual redness, after completion of the AC protocol.
WRA/Matrix IR may be applied to old scars in the same session of AC, if skin response allows.
The inflamed acne lesions have healed, but some redness persists. It may be taken care of by SR/SRA at the end of the acne protocol.
Complete healing.
The residual acne scars may be treated by Matrix IR or WRA applicators.
The hair shaft, is shaved, not to waist the energy upwards. Following treatment, the detached hair shaft is gradually shed off (within 1-3 weeks) and is rarely singed. The cavity is later filled with fibrotic tissue.
The shaft starts growing from cells at the bottom of the bulb around the dermal papilla, where also melanin is deposited into the cells. This growth is regulated by cells at the bulge, which is at the top area of the bulb, just where the muscle connects into the follicle.The bulge gives rise to a new bulb every cycle.
The cells at the bulge give rise to a new bulb in the transaction from telogen to anagen. Germinative cells at the bottom part of the new bulb give rise to a new hair shaft.
In anagen, the full hair shaft with a lot of melanin is present for maximal absorption.
Catagen is a short transient phase.
In telogen, most hair shafts will be shed off. Even when present, the contact between them and the follicular cells is very loose. Only a few germinative cells are left to give rise to a new bulb, and the rest of the bulb is degenerated.
Following a few treatments, there is a certain degree of synchronization, so more hair are in the same stage of the cycle and a higher percentage of hair may be removed in each subsequent session.
The hair shaft mediates between the optical energy source and the hair follicle, as the heat produced in the shaft after light absorption, is conducted to the hair follicle.
As electrical current that travels between two electrodes cannot travel though high resistance objects. The hair shaft (keratin) is one of these and as RF energy comes in contact with the hair it concentrates in the follicle which has a better electrical conductivity than the shaft and after pre-heating by the optical energy gains more electrical conductivity relative to the surrounding tissue.
When the hair is pulled out after treatment, its damage is more pronounced after combination treatment with IPL and RF as compared to IPL alone.
White hair depends on residual melanin in the bulb, that cannot be apparent. Therefore, white hair may or may not respond to treatment. Mixed white and grey hair stand a better chance to have residual melanin in the bulb.
Blond hair on very light skin may cause light reflection. The darker skin may absorb more optical energy for collateral heating. However, the skin should not be darker than the hair.
When multiple passes are attempted, it is important to keep the treatment area at a limited size (<100 pulses), to facilitate heat accumulation.
Light hair may show less clearance and may require more sessions than dark hair. Regarding white hair, even though there are a few papers about clearance, it is not predictable and depends probably on the amount of follicular residual melanin which cannot be estimated.
Villous hair, such as on women’s cheeks and neck are too thin to be treated. Even the highest parameters are not sufficient and may in fact induce the growth of darker and coarser terminal hair by bio-stimulation.
Emphasize FDA terminology of “permanent hair REDUCTION” vs. permanent hair REMOVAL. Used to be “long term hair removal”.
When there is only partial destruction of the hair follicle, there will be a delayed growth of finer and lighter hair. This in turn is more difficult to treat than originally. Therefore, in hair removal application, more than in any other application, it is better not to be too cautious with treatment parameters.
We are born with a set amount of follicles (~5 million). Only a portion of it is cycling through anagen/telogen. Dormant pool may be activated by physiological changes, such as hormonal changes and trauma.
The hair that grows in the treated area is from different follicles to those that have been destroyed. Therefore we cannot promise permanence, neither 100% clearance, as the client does not distinguish between different follicles on the same area. Explanation of this issue is very important!
The aim of this treatment was thinning the hair. For better clearance, more sessions are necessary with long intervals, up to 15 weeks.
White hair depends on residual melanin in the bulb, that cannot be apparent. Therefore, white hair may or may not respond to treatment. Mixed white and grey hair stand a better chance to have residual melanin in the bulb.

If there is no evident response and no change in skin, additional pass may be performed. When additional pass is attempted on areas no larger than 100 pulses, same pulse type and RF may be used with the following optical parameters: same for skin types I-III and 10-20% lower for IV-VI. Additional 2 passes on light/fine hair has to be in Long pulse type for all skin types.
For Skin Type V, use the low end of the range with Short and the higher with Long pulse.
The aim of this treatment was thinning the hair. For better clearance, more sessions are necessary with long intervals, up to 15 weeks.
