1. Dry socket

3. 1. Dry socket
– It is due to loss of
blood clot and
presence of exposed
bone in the socket
– Analgesics
ineffective
– Fetid odor from the
mouth

4. Dry socket
• Treated by:
– Anesthesia
– Irrigation
– Pack socket with gauze
+ Eugenol
– Analgesics
– Repeat after 24hrs until
symptom-free, and
then remove packing

5. Dry socket instructions
• No excessive spitting
• No vigorous rinsing
• No drinking through straw
• Avoid tobacco use

• Infection may persists
at the time of
extraction or it might
develops after
extraction due to the
presence of perapical
infection
• Treated by antibiotics

7. 3. Prolonged bleeding
• Bleeding
– Normal oozing 6-12
hours post operative
– Maintain pressure
with good packing
Clot formation

8. 3. Prolonged bleeding
• If bleeding continue a problem might exists
• E.g. presence of infected tissue at the site of
extraction, patient on Aspirin, or platelets
dysfunction…
• Treatment: clean the socket and apply
pressure until hemostasis obtained

Causes:
– Some post operative swelling (edema)
can be expected due to trauma of
surgery
– Usually resolves 24-36 hours post op
– Swelling after 36 hours may indicate
infection

11. 5. Displacement of tooth or part of
tooth into the maxillary sinus
Upper teeth only
• In such cases, almost always the tooth or tooth
fragment must be retrieved
• In some cases, the sinus cavity can be irrigated
with saline and the tooth fragment may be
brought back to the site of the opening through
which it entered the sinus, and may be
retrievable
• At other times, a window must be made into the
sinus in the canine fossa for tooth removal

13. 6. Nerve injury
• Mainly during extracting third molars
• Two nerves are typically of concern:
– The inferior alveolar nerve
– The lingual nerve
• Such injuries are usually temporary, but
depending on the type of injury the damage
can be prolonged or even permanent

• If patient is already on analgesics
• Reassure the patient
• Make sure no infection, dry socket…
