....เหตุผลก็ยอนกลับไปสมัยเรี ยนจบใหม่ เพราะฉันเองเป็ นเพียงหมอจบใหม่ที่

แพทย์ 5-6 ปี นัน อาจารย์ และพีๆจะพยายามถ่ายทอดวิ ชาเพื อให้เราสามารถพร้อมเป็ นหมอที ดูแลผูป่วยได้ แต่ความ

จริ งก็คือมี แพทย์ จบใหม่จานวนไม่นอยยังมี ความรู้และประสบการณ์ ไม่พร้อมในการดูแลผูป่วย ตัวฉันเองก็เช่นกัน

แม้ว่าจะเป็ นบัณฑิ ตเกี ยรติ นิยมจากสถาบันเก่าแก่...แต่ฉนก็กลับรู้สึกว่าฉันเองยังขาดทักษะและความรู้อยู่(เพือนคนที ่

หยิ บติ ดมื อไปด้วยทุกครั้งที ไปตรวจหรื ออยู่เวร บ่อยครั้งที ตองเปิ ดหนังสือซึ่งนันก็ช่วยฉันได้หลาย

ครั้ง แต่หลายครั้งก็พบว่า ไม่มีเนือหาที ตองการ ไม่มีเนือหาที ่ทาให้รักษาผูป่วยได้ ....และนีคือแรง

การรัดคอมีได้ หลายแบบ คือ
1.การแขวนคอจากที่สง (hanging)

4.การรัดคอโดยอาศัยท่าทาง
(postural strangulation)
กลไกที่ทาให้ คนตายจากการรัดคอมีอยู่ 3 อย่ างคือ
1.มีการบาดเจ็บของไขสันหลังและก้ านสมอง
2.มีการบีบรัดในส่วนของโครงสร้ างร่างกายบริ เวณคอ : หลอดลมและเส้ นเลือดดาหรื อแดง
3.หัวใจหยุดเต้ น
Hanging : การแขวนคอจากที่สูง มีสาเหตุท่ ทาให้ เกิดความตายได้ ดังนี ้

1.ถ้ าแขวนคอแบบเท้ าไม่ติดพื ้น (complete hanging) อาจจะตายจาก hangman's fracture หรื อมีการฉีกขาดของไขสัน
หลัง (transection of the spinal cord) ทาให้ หยุดการหายใจ ซึงสองอย่างข้ างต้ นทาให้ เสียชีวิตในเวลาอันรวดเร็ว หรื อกด

blunt chest injury แบ่งออกเป็ น 3 ประเภทใหญ่ๆ
(1)chest wall fractures, dislocations, and barotrauma (e.g., rib fractures, flail chest, sternal fractures,
diaphragmatic injuries)
(2) blunt injuries of the heart, great arteries, veins, and lymphatics (e.g., myocardial contusion, aortic
disruption, cardiac rupture/ tamponade),
(3) blunt injuries of the pleurae, lungs, and aerodigestive tracts (e.g., pulmonary contusion, hemothorax,
pneumothorax, tracheobronchial disruption, ruptured esophagus)
เมื่อสงสัยว่ามี blunt chest injury ให้ ดู V/S ผู้ป่วยว่า stable หรื อไม่ ถ้ าไม่ ให้ resuscitate primary +
secondary survey เบื ้องต้ นไปก่อน พร้ อมกับหาว่ามีการบาดเจ็บใน 3 แบบข้ างต้ นหรื อไม่ โดยการตรวจร่างกาย และ
ตรวจเพิ่มเติม เช่น CXR, (+/- CT chest ในบางกรณี) เมื่อพบความผิดปกติ จึงจะมีการตรวจเพิ่มเติมละเอียดต่อไป เช่น
CBC count(ใช้ ประเมิน blood loss), ABG(ใช้ ประเมิน ventilation, oxygenation,และ acid-base status),
Coagulation profile ในคนที่ต้องได้ รับเลือดจานวนมาก(>10 U packed RBCs), ECG,Serum troponin, Serum
myocardial muscle CK isoenzyme levels ในคนที่สงสัย myocardial injuries, Thoracic ultrasound ในรายที่สงสัย
Pericardial effusions or tamponade, Aortogram ในรายที่สงสัย thoracic aortic injuries , Esophagoscopy ในราย
ที่สงสัย esophageal injuries(ถ้ าทาscope แล้ ว negative อาจทา Contrast esophagogram เพิม) โดยในผู้ป่วยที่อาการ
่
หนัก V/S unstable อาจจาเป็ นต้ องได้ รับการผ่าตัดอย่างเร่งด่วน ไม่รอ W/U ละเอียด

ที่มี pulmonary contusions.
**ในกรณี Fx rib ทียงไม่ถึงเกณฑ์ massive flail

การใส่ arm sling หรื อ shoulder harness แต่ถ้ามีการ
เสียหายที่ glenoid, acromion, หรื อ coracoid อาจ
ต้ อง open reduction and internal fixation
ตรวจพบมีวาไหล่จะบวมและมี

ตรวจร่างกายอาจพบ pericardial ส่วนใหญ่ blunt pericardial injuries รักษาเย็บซ่อม
Isolated blunt pericardial
rub.
แบบ simple pericardiorrhaphy แต่ถ้า Large
injuries are rare. Blunt

Management
ถ้ าเป็ น Open pneumothorax (มีแผลรูเปิ ดที่ chest wall ต่อถึง pleural space) ให้ ใช้ Vaseline gauze seal ที่แผล
ก่อน (three side dressing) และใส่ ICD ทุกราย, อย่าลืม control pain
ถ้ าเป็ น traumatic pneumothorax ธรรมดา ใส่ ICD ในรายที่มีปอดแฟบมากกว่าร้ อยละ 50 หรื อเกิดภาวะ
hemopneumothorax
Hemothorax
อาจเป็ นจาก spontaneous หรื อ traumatic ก็ได้
Clinical presentation
History : pleuritic chest pain, dyspnea
Investigation
X-ray : air fluid level
Physical examination : เคาะทึบ, ฟั ง decrease breath
sound ข้ างนัน (โดยถ้ า Massive Hemothorax ผป.อาจจะมี

shock และเมื่อใส่ ICD พบมีเลือดออก >1500 ml in 2 hr,
>1000 ml in 1 hr หรื อ >200 ml/hr ติดกัน 3 hr)
Management (1) การรักษาเบื ้องต้ น คือ resuscitation ด้ วยสารน ้า ในกรณีที่เสียเลือดมากให้ จองเลือดด้ วย, O2
(2) ใส่ทอ ICD** (32-34 F) เพื่อระบายเลือดออกจากช่องเยื่อหุ้มปอด และหากยังมีเลือดออกต่อเนืองหรื อไม่สามารถ

ระบายออกได้ หมดทาให้ ปอดไม่สามารถขยายได้ เต็มที่ก็ต้องผ่าตัด (off ICD ได้ เมื่อเลือดหยุดออกตามท่อแล้ ว)
(3) +/- ผ่าตัด ซึงมักจะทาในวันที่ 7-10 หลังจากเกิด hemothorax ซึงการผ่าตัดจะเป็ นการเอาก้ อนเลือดออกร่วมกับลอก

เปลือก fibrin ที่ห้ มรัดอยูบนเนื ้อปอดเพื่อให้ ปอดขยายได้

(4)รักษาพยาธิสภาพที่ตรวจพบว่าเป็ นสาเหตุตอไป

ผ่าตัดทันทีมี 2 อย่าง คือ peritonitis / diffuse abdominal tenderness และ hypotension ที่ไม่สามารถอธิบายได้ จาก
การบาดเจ็บอื่นๆ
Initial Evaluation
ผู้ป่วย blunt abdominal trauma มักมีความยุงยากในการวินิจฉัยมากกว่า penetrating abdominal
