Những bào quan:
Peroxisome, ty lạp thể (mitochondria)
Lysosome.
Hạt đặc điện tử (electron dense granule)
Hạt đặc hiệu α (specific α-granule)
Các bào quan của tiểu cầu ngoài chức năng thông
thường còn chứa rất nhiều yếu tố như: các yếu tố
tham gia vào quá trình đông máu, yếu tố điều biến
tăng trưởng…(do TC sản xuất hoặc hấp thu).

Các receptor của tiểu cầu: TC có khoảng 10 nhóm với hàng trăm
loại Receptor
1. GPIa cũng như phức hợp GPIa/IIa: là những vị trí gắn kết
với sợi collagen của lớp dưới nội mạc khi nó bị bộc lộ ra.
2. GPIb và phức hợp GPIb-IX-V là receptor nhận biết chuẩn bị
cho dòng thác hoạt hoá GPIIb-IIIa
3. GPIIb/IIIa (intergrin αIIbβ3). với fibrinogen và vWF tạo
thành lưới tiểu cầu
4. P2-Receptor: P2Y12 chịu tác động của ADP và P2X1 chịu
tác động của ATP.

Vai trò của tiểu cầu trong huyết khối-vữa xơ
Vữa xơ mạch máu gây tăng hoạt hóa và kết dính TC
hình thành cục máu đông. Ngược lại tiểu cầu hoạt hóa cũng làm
giải phóng hàng loạt các mediator viêm, yếu tố đông máu và co
mạch thúc đẩy quá trình phát triển của huyết khối vữa xơ
Có sự gia tăng hoạt hoá tiểu cầu sau khi tắc mạch não ngưng
kết tiểu cầu ở các vi mạch trong vùng thiếu máu gây mất dòng
chảy ngược (no-reflow).

Transient Ischemic Attack (2010)
For patients with stroke or TIA due to 50% to 99% stenosis of a major
intracranial artery, aspirin is recommended in preference to
warfarin (Class I; Level of Evidence B). Patients in the WASID trial were
treated with aspirin 1300 mg/d, but the optimal dose of aspirin in this
population has not been determined. On the basis of the data on general
safety and efficacy, aspirin doses of 50 mg to 325 mg of aspirin daily are
recommended (Class I; Level of Evidence B). (New recommendation)
For patients with noncardioembolic ischemic stroke or TIA, the use of
antiplatelet agents rather than oral anticoagulation is recommended
to reduce the risk of recurrent stroke and other cardiovascular events
(Class I; Level of Evidence A).

For patients unable to take oral anticoagulants, aspirin
alone (Class I; Level of Evidence A) is recommended. The
combination of clopidogrel plus aspirin carries a risk of
bleeding similar to that of warfarin and therefore is not
recommended for patients with a hemorrhagic
contraindication to warfarin (Class III; Level of Evidence

50. ÁP DỤNG ĐIỀU TRỊ
Trong điều trị nhồi đột quỵ thiếu máu não cấp
+ Các thuốc chống kết tấp tiểu cầu nên được sử dụng sớm nếu
không có chỉ định điều trị rtPA
+ Aspirin đã được khẳng định có hiệu quả giảm được khoảng 10
bệnh nhân tử vong hoặc tái phát, tăng khoảng 9 bệnh nhân hồi
phục hoàn toàn trên 1000 ca điều trị
+ Các thuốc khác như Clopidogrel, Aggrenox… cũng cho thấy
an toàn và có hiệu quả tương đương hoặc hơn Aspirin nhưng chỉ
mới giới hạn trong một số nghiên cứu nhỏ.

+ Aspirin giảm nguy cơ tương đối (RR) 13% vs giả dược
+ Clopidogrel giảm nguy cơ tương đối RR 9% (0,3-16%) vs aspirin
+ Aspirin +ER dipyridamole RR 18% (9-26%) vs aspirin
+ Clopidogrel tương đương aspirin + ER dipyridamole
+ Không khuyến cáo kết hợp Aspirin + Clopidogrel thường quy
Có thể áp dụng cho bệnh nhân có hội chứng vành cấp hoặc đặt stent
