Objectives:
1. Refresh teacher’s knowledge on accomplishing school
forms
2. Update teachers with the changes on accomplishing school
forms
Expected outcomes:
1. Accomplish given school forms correctly
2. What are the common school form
that we accomplish in the school?
• School Register ( Form 1 )
• Teacher’s Monthly Report on Enrolment
and Attendance ( Form 2 )
• Report on Promotion ( Form 18-E )
• Elementary School Permanent Record
( Form 137-E )
• Pupil’s Report Card ( Form 138-E )
3. Form 1 - School Register
• should have been accomplished by July
2013 (every school year )
• transfer in/ late enrollees should be
written after the last entry and will not be
included in the alphabetical order.
• Monthly attendance should be
accomplished regularly.
5. Form 1 - School Register
• Age for June (opening) should be
referred to the reference table at the
back of the form 1
• Care should be taken in using the table
for determining ages.
• Require the pupils/ parents for a copy of
their birth certificate for the correct
birthdate
7. Form 1 - School Register
• Total enrolment for the year to date is
your annual enrolment ( July )
• Total enrolment for month to date is your
latest monthly enrolment including the
transferees in / excluding the transferred
out / dropped out
• Daily attendance should be filled up
regularly

• Total number of attendance – number of
absences

• Fill up the Total enrolment for the year to date
• Fill up the enrolment for the month to date
• Attendance :
enrolment x number of days – number of
absences ( for the month )
Example:
19 pupils x 21 days - 7 absences = 392

• Form 1 and
Form 2 are
sisters.

Enrolment and Attendance
• Average Attendance:
Add the total column under “Daily Attendance” and divide with
the total number of session/ days for the month.
• Percentage of Attendance: Average attendance x 100 / monthly
enrolment
• Percentage of Enrolment: Monthly enrolment x 100 / Yearly
enrolment

13. Report on Promotion / Elementary School
Permanent Record
( Form 137-E )/ Pupil’s Report Card ( Form
138-E )
• Correctness of Pupils Information Data –
names, birthday, birth place, name of parents
• Avoid erasures especially on Permanent
Record (Form 137-E) and Report Card (Form
138-E).
• Attach a certified copy of birth certificate to the
Permanent Record (Form 137-E)
• All Forms should tally with each other.
14. Remember:
Experience is a hard teacher
because she gives the test
first, the lesson afterward.
