1. Introduction
This Environment Monthly Report is provided by PT Gunanusa Utama Fabricators is order
to ensure the measurement of the consistency on implementing HSE Management System,
Legal requirements and HSE Policy related to environmental issues which might exist and
rise at workplace during the execution of BTJT-A Topside Modification, Adaptor Fabrication
and Pile Make Up For Bukit Tua Development Project in CTR Yard. CTR recognized the
importance of planning Control Measures into construction activities at the earlier possible
stage in order to prevent damage to the Environment. This Construction Yard and Site
Environment Management Report Plan has been produced to describe how to CTR intends
to manage, control, and coordinate its work, to ensure that no injury, to personal as result of
environmental related activities and no damage or loss shall occur to the environmental as
result of the construction activities. This report consists of summary of implementation of
PSP-BTJT-A-T011 Health, Safety and Environment Program that already approved by
Company
2. Purpose
The purposes of this report are as follows:
1. Identify all the operational activities that presume and may cause important and major
impact to environmental component.
2. Identify the environmental aspects especially presumed the environmental that may
contaminate by the important and major impact of activities.
3. Predicated and evaluated the important and major impacts that happen toward
environmental component because of the operational activities.
4. Mitigation the important and major impacts through environmental component.
5. To choose the exactly and realistic effort of environmental monitoring and management
efforts to minimize the negative impact and increase the positive benefit.
3. Scope
This report covered of all CTR activity on implementing and managing the environmental
requirements at the CTR Yard area on BTJT-A Topside Modification, Adaptor Fabrication
and Pile Make Up For Bukit Tua Development Project and CTR facilities such as: waste
register, environmental monitoring and inspection, environmental improvement,
environmental planning, environmental training, other environmental issues

5. Environmental Statistic
CTR will give efforts to received environment objectives such as: to ensure no oil spill and to
minimize dust exposure during sandblasting, and improve waste management. Therefore
with environmental statistic the environment objectives will be monitored.

6. Environmental Induction, Communication and Awareness Program
Environmental induction has been given to each and every new employee which done
during HSE Induction. Environmental awareness was promoted continuously to the workers
through toolbox talk, mass meeting at work locations, and training, and Environment
Posters at HSE notice board for increased the knowledge about environmental and healthy
habit.
Title of Toolbox Topic in September 2014 is about “Housekeeping at Warehouse”.
Attendance List refer to ATTACHMENT 1.
7. List of MSDS
List of MSDS of all the chemicals being in use at CTR Yard is available at office, clinic and
yard area during BTJT-A Topside Modification, Adaptor Fabrication and Pile Make Up For
Bukit Tua Development Project. The MSDS currently available are:

8. Activity and Area Description
Following were the work locations at CTR Yard during BTJT-A Topside Modification,
Adaptor Fabrication and Pile Make Up For Bukit Tua Development Project work activity in
the September 2014:

Waste is segregated through the different type of waste bins as per approved GNU-P-SH-
9005 Waste Management Plan. At CTR Yard area using 5 colors for each type of waste,
such as:
 Yellow : Burnable Waste (Paper, Cartoon, Wooden, etc)
 Green : Unburnable Waste (Plastic, Rubber, etc)
 Blue : Organic Waste
 Red : Metal Scrap
 Black : Hazardous Waste

The domestic waste storage at CTR Yard and will transport by small truck to waste collector
(CV. Mandiri Pratama Abadi). Hazardous waste from CTR Yard will be sent to the third
party. Waste transfer note is recorded by HSE Department from CTR. Waste register has
been from the waste transfer note individually for Yard. Summary of waste transfer note
attached on ATTACHMENT 2 and total quantity of waste generated per month from yard is
entered in the register (ATTACHMENT 3).

10. Environmental and Hygiene Inspection
Environmental and Hygiene Inspection have been performed in CTR Yard area during
Yadana Subsidence Project regularly. Environmental and Hygiene Inspection were done in
this period such as:

11. Environmental Monitoring
11.1 Drinking Water Quality
Drinking water supply for CTR Yard is supply from CTR Aqua/Water Maker. Drinking water
quality analyse was conducted on September 17, 2014 by Health Laboratory Department
Serang region. The checked parameter of drinking water was based on Health Minister
Regulation No. 492/MENKES/Per/IV/2010 regarding requirements and water quality
monitoring. The result is shown that drinking water from CTR Yard is safe to use. Report
related this matter can be seen on ATTACHMENT 5.
11.2 Waste Water Quality
Waste water monitoring conducted every six-months. Waste water quality sampling was
taken from drainage as domestic waste water or grey water at CTR Yard near Jetty 1 on
April 25, 2014, by Health Laboratory at Serang, will conduct as per quarterly basis. Analysis
based on Decree of Environmental Minister No. 51 Year 1995 regarding Standard of
Industrial Waste Water. The result of waste water sample was shown that waste water
saved to discharge to the sea. The result of monitoring refers to ATTACHMENT 6.
11.3 Air Quality
HSE department of CTR has done some efforts to minimize the effect from dust and
chemical pollution on air, such as:
 Installation of dust screen at Manual Blasting and Painting area
 Provide proper paint storage and mixing paint area

 Provide PPE for employees (Masker)
 Conducted regular induction for new employee and Toolbox Talk as per requirement
Air Quality Monitoring at CTR Yard conducted every six-months. Air Quality Monitoring with
chemical (SOx, NOx, H2S, NH3, O3) and dust parameter already conducted by Health
Laboratory Department Serang region on June 2014 for Workshop 2 area. Analysis of air
quality monitoring based on Decree of Environmental Minister No. 50 Year 1996 regarding
Standard of Odour and Indonesia Government Regulation No. 41 Year 1999 regarding Air
Pollution Prevention. The result of Air Quality Monitoring result was shown that air quality
monitoring with chemical parameter at Workshop 2 area below than threshold limit. The
result of monitoring refers to ATTACHMENT 7.
