CONTRACTOR shall ensure that all personnel are properly

CONTRACTOR shall maintain ratio of anomaly reported per
million man-hours above target fixed by COMPANY.

8. HSE INCENTIVE SCHEME 3
1. SHOC form should be socialized in every TBM/Project Mass Meeting by
Safetyman/Safety Officer/HSE Coordinator (HSE Personnel).
2. The minimum SHOC form are 25 copy sheets per HSE Personnel and it should be given
for all of worker per work area.
3. SHOC form should be collected base on minimum target 100 SHOC in every week and
every HSE Personnel.
4. SHOC Analyzed should be prepare daily SHOC form minimum are ± 300 copy per day.
