CFU = COLONY-FORMING UNITS
Chain of Transmission
Contact with water is the #1
antagonist of healthy skin
Action: Prolonged
washing destroys the
protective function of the
stratum corneum – natural
skin oils are dissolved and
washed off.
Result: irritant contact
dermatitis can develop.1) natural skin oils
2) skin surface
3) water
Affects of Washing
Normal, healthy skin
Cracked skin
Affects of Washing
Cracked and scaly skin
provides microorganisms
ideal niches
Antiseptics cannot reach
hidden non-socomial
pathogens
Hand disinfection is in
danger of not being
effective enough
VAST MAJORITY of situations
in clinical practice calls for
hand disinfection - not hand
washing!
Affects of Washing

CDC on Hand Hygiene
• Considerations when selecting appropriate antiseptic agents
Essential performance characteristics (i.e., spectrum and persistence of
activity and whether the agent is fast acting)
Reliable vendor, support, and supply
The needs of the health care professional (i.e., allergies, compatibility with
lotions, scents, etc.)
Alcohol based rubs should contain 60%-95% ethanol or isopropanol

Editor's Notes

  1. Given the patient safety goals, we must address the issues associated with hand hygiene and the role it plays in disease transmission. It is well documented that good hand hygiene protocols are the first and best defense against healthcare associated infections (HAIs). The reason this is so is because disease is so easily transmitted via the hands. The chain of transmission depicted here between the patient, the clinicians and inanimate objects is never-ending. The only way to break this chain is to interrupt it via hand disinfection. And, we must do so without jeopardizing the skins integrity because damaged skin yield yet another venue for infection.
  2. But, the by its very nature, hand washing with soap and water is quite often the most damaging thing you can do to your skin. When the natural oils inherent in our skin is removed via friction and washed away via rinsing, we are left with less protection and it is quite common to develop irritant contact dermatitis.
  3. And, when this dermatitis does manifest itself, we invite infection. As you can see here, damaged skin provides pathways for bacteria to penetrate beyond where antiseptic agents like antibacterial soap - or even alcohol-based hand rubs - can penetrate. So, if our skin is damaged by excessive use of soap and water, it may very well be that hand disinfection with any antiseptic is simply not enough. And, while most clinical situations now call for the use of alcohol based hand rubs over soap and water, we can see how we might be vulnerable if our skin is compromised.
  4. So, when we speak of good hand hygiene protocols, we must speak of the three things that the CDC and all respected experts agree upon and that are required: Cleansing (via hand washing of visible soil) , Antisepsis or disinfection and general Skin Care with moisturizing lotions.
