3. Checking
• Before Installing PIDSR VERSION 4,
Check Your Computer for previous
installations of:
– EPI INFO FOR WINDOWS
– PIDSR
– WINZIP
If they exist, Uninstall all EPI INFO, PIDSR and
WINZIP applications.
4. Back-up
• If the 3 application exist back-up your Current
Folder to keep your previous years’ MDB Files.
1. To Back-up, copy the whole Current folder
found in this path – C:PIDSR
2. Paste the Current folder to any drive you want
or burn the files in a disk.
Ex. Paste to this path – E:Back-up2011 MDB
FILES
3. Rename the folder for identification
Ex. 2011 MDB FILES
5. Uninstalling
1. Click start
2. Choose All Programs
3. Look for Epi Info
4. Then click uninstall Epi Info
5. Follow the instructions…ok….ok
6. Please do the same steps in uninstalling
PIDSR and WINZIP.
6. Installing PIDSR
1. Insert Flash Drive or CD containing the
PIDSR 4 Installer
2. Look for the drive for the Flash drive or the
CD Drive
3. Open the Folder of the Installer
4. Double click setup.exe
5. Follow the instructions
6. To select destination folder please select
C:PIDSR
7. Click FINISH when done.
7. Installing EPI Info
1. Look for EPI Info in
this path
C:PIDSREpi Info
2. Double click Epi
Info.exe
3. Follow the
instructions
4. To select destination
folder please select
C:Epi_Info
5. Otherwise click the
browse button.
8. Cont……Installing EPI Info
6. A dialog box will appear to allow you to
change the destination folder as shown
below.
9. Cont…..Installing EPI Info
7. Then in the Folder name box change the
drive letter to C to make it C:Epi_Info as
illustrated above then click OK. Just keep
clicking Next for the following steps and
CLICK FINISH WHEN DONE.
10. Installing WINZIP
1. Look for the file
WINZIP81 in this path –
C:PIDSRWINZIP81
2. Double click on
WINZIP81.exe
3. Click Setup
4. Follow the succeeding
instructions.
5. On the destination
folder, always specify
drive C: so that it reads
“C:Program
FilesWinZip”
6. Click FINISH when done.

2. Choose Enter Data
3. Choose a Disease
4. Click New (First Icon – Upper left portion of the
Screen)
5. Start entering Data using your CIF or CRF.
6. The upper portion of the sheet refers to the
Reporting Unit Information, while the lower
portion refers to the patient information.
7. Name of DRU – Disease Reporting Unit, choose a
DRU at a drop down box. For RHU, PHO and CHO
follow this format: Ex. Mambusao RHU, Roxas CHO,
and Capiz PHO
8. Complete entries especially those with asterisk*
