  8. 8. Medical Excuses <ul><li>Parent Notes </li></ul><ul><ul><li>By state law a parent may request an excuse for up to three consecutive days. A written note from a parent or guardian must be turned into the instructor on the day of the excuse . The note is only good for that day and must be dated and signed with a phone number </li></ul></ul><ul><ul><li>Non-participants will be required to “DRESS-OUT” and observe activities along with an academic assignment. </li></ul></ul><ul><li>Medical Notes </li></ul><ul><ul><li>After three days, a physician’s note is needed and will be honored for as long as the doctor designates. </li></ul></ul><ul><ul><li>Students will not be required to dress-out </li></ul></ul><ul><ul><li>Effort points may be awarded for completing a special assignment </li></ul></ul><ul><ul><li>Student may be sent to the library to work on the special assignment after reporting to the teacher for attendance. </li></ul></ul>
