2. It all depends what your Appraisal Form looks like - remember an individual was hired to do a
specific job function - the job outline needs to cover all aspects of the persons job function the - WHAT, HOW, MEASUREMENT, PERFORMANCE EVIDENCE and WHEN
An appraisal form is not worth the paper it is written on if it doesn't correspond directly to the
Job Outline of an individual, with the a comprehensive MEASUREMENT criteria and
PERFORMANCE EVIDENCE to back it up - the WHEN is also very important - it is pointless
measuring an individual MONTHLY - when there is not enough evidence to prove or disprove
the individual is performing to the required standard

If your form is not working - I doubt just rewriting the form will help you need to look at all of
the aspects mentioned above...

Remember the Appraisal form is only one aspect of the bigger picture - I recommend you do
some research on complete Performance Management SYSTEMS....
