• 1876 Joseph Lister người đầu tiên
công bố việc sử dụng carbolic acid
(phenol) làm chất sát khuẩn trong
phẫu thuật.
• Các chất sát khuẩn nên được áp
dụng trên da vùng phẫu thuật và
xung quanh để giảm thiểu vấy
nhiễm, bảo vệ tính toàn vẹn của da
và ngăn ngừa tổn thương mô.

3. Những NK tại vị trí PT trong thời gian từ khi
mổ cho đến 30 ngày sau mổ với PT không có
cấy ghép và cho tới một năm sau mổ với PT
có cấy ghép bộ phận giả, chia thành 3 loại:
• NKVM nông: NK ở lớp da hoặc tổ chức
dưới da tại vị trí rạch da;
• NKVM sâu: NK tại lớp cân và/hoặc cơ tại
vị trí rạch da. NKVM sâu cũng có thể bắt
nguồn từ NKVM nông để đi sâu bên
trong tới lớp cân cơ;
• Nhiễm khuẩn cơ quan/khoang cơ thể

Các phẫu thuật không có nhiễm khuẩn, không mở vào đường hô hấp, tiêu
hóa, sinh dục và tiết niệu. Các vết thương sạch được đóng kín kỳ đầu
hoặc được dẫn lưu kín. Các phẫu thuật sau chấn thương kín.

Sạch

Các phẫu thuật mở vào đường hô hấp, tiêu hoá, sinh dục và tiết niệu
trong điều kiện có kiểm soát và không bị vấy nhiễm. Trong trường hợp
đặc biệt, các phẫu thuật đường mật, ruột thừa, âm đạo và hầu họng được
xếp vào loại vết mổ sạch nhiễm nếu không thấy có bằng chứng nhiễm
khuẩn/ không vi phạm nguyên tắc vô khuẩn trong khi mổ.
5 – 10
Nhiễm
Các vết thương hở, chấn thương có kèm vết thương mới hoặc những
phẫu thuật vi phạm nguyên tắc vô khuẩn hoặc phẫu thuật để thoát lượng
lớn dịch từ đường tiêu hoá. Những phẫu thuật mở vào đường tiết niệu
sinh dục, đường mật có nhiễm khuẩn, phẫu thuật tại những vị trí có nhiễm
khuẩn cấp tính nhưng chưa hoá mủ.

Bẩn
Các chấn thương cũ kèm theo mô chết, dị vật hoặc vấy nhiễm phân. Các
phẫu thuật có nhiễm khuẩn rõ hoặc có mủ.

9. Guidelines WHO 2016
• Các nghiên cứu gần đây của WHO ghi nhận:
◦ NKVM là loại nhiễm khuẩn liên quan chăm sóc y tế (HAI) được khảo sát nhiều nhất
◦ Loại Nhiễm khuẩn liên quan chăm sóc y tế thường gặp nhất ở các nước thu nhập
thấp và trung bình
◦ Ảnh hưởng đến một phần ba số người bệnh được phẫu thuật.
• Mặc dù tỷ lệ NKVM thấp hơn ở các nước thu nhập cao, nhưng đây vẫn là
loại nhiễm khuẩn liên quan chăm sóc y tế thường xuyên nhất ở châu Âu
và Hoa Kỳ.

10. Phương pháp xây dựng guidelines
1. Xác định các kết cục quan trọng chính, các chủ đề ưu tiên và xây dựng
một loạt câu hỏi cấu trúc dưới dạng PICO (Population, Intervention,
Comparison, Outcomes: Dân số, Can thiệp, So sánh, Kết quả);
2. Truy xuất các chứng cứ thông qua các đánh giá có hệ thống cụ thể của
từng chủ đề sử dụng một phương pháp đã được chuẩn hóa;
3. Đánh giá và tổng hợp các chứng cứ;
4. Xây dựng các khuyến cáo;
5. Viết hướng dẫn, lập kế hoạch phổ biến và chiến lược thực hiện liên quan
11. Guidelines của CDC 2017
• CDC Hoa kỳ đã ban hành các khuyến cáo dựa trên chứng cứ cập nhật
nhằm phòng ngừa NKVM.
• Bao gồm 14 lĩnh vực chính và dự kiến sẽ phối hợp với các chương trình
cải tiến chất lượng phẫu thuật để đảm bảo an toàn cho NB (ERAS ...).
• Các khuyến cáo năm 2017, được ban hành trực tuyến ngày 3/5/2017 trên
tạp chí JAMA, thay cho hướng dẫn NKVM của CDC 1999 đã được xuất
bản trước khi việc phân loại dựa trên chứng cứ được thường qui áp dụng

17. Antiseptic Prophylaxis
• Advise patients to shower or bathe (full body) with soap (antimicrobial or non-
antimicrobial) or an antiseptic agent on at least the night before the operative
day. (IB–strong recommendation.)
• Perform intraoperative skin preparation with an alcohol-based antiseptic agent
unless contraindicated. (IA–strong recommendation; high-quality evidence.)
• Application of a microbial sealant immediately after intraoperative skin

◦ Phổ khuẩn trên người bệnh (da, niêm mạc, đường tiêu hóa),
◦ Từ một ổ nhiễm khuẩn chỗ khác trong cơ thể.

The Global Alliance for Infections in Surgery is an independent alliance promoted in 2016 by the World Society of Emergency Surgery (WSES), for a cohesive and global approach to promoting research and education in the field of infections in surgery.
Nhiều nghiên cứu cho thấy rằng, các hợp chất iodophor là chất sát khuẩn hiệu quả nhất. Iode kết hợp với các chất tác động bề mặt hữu cơ, chẳng hạn như polyvinylpyrolidon (Betadine, Isodine). Chris Barys, General Manager, I-Flow, a Kimberly-Clark Healthcare Company, Lake Forest, CA, talked specifically about their surgical sealant. "Kimberly-Clark addresses a safer environment for surgical patients through InteguSeal Microbial Sealant, a sterile, film-forming liquid that is applied to the skin over commonly used surgical skin preparation products prior to a surgical incision to reduce the risk of both surgical-site infections and microbial presence in high-risk patients. The technology has been shown to seal and immobilize skin flora that preps miss – including methicillin-resistant Staphylococcus aureus (MRSA), Staphylococcus epidermidis, and Escherichia coli–keeping them from migrating into the surgical incision."
InteguSeal Microbial Sealant is effective throughout the surgical procedure; is breathable; easy to apply; fast-drying; and comes in three sizes. Cost of using InteguSeal is approximately $30 per procedure.
