I would like to express the vision on safety in Pak Suzuki, the purpose of
which is to ensure a safe and healthy working environment for the
prevention of injuries and illness. We follow Occupational Health and Safety
Management System in our work place and comply with relevant legislation.
Being a leading Automobile Assembler in Pakistan, the
management of Pak Suzuki gives the core importance to safe
work practices, protection of the environment and the health of
its employees to conduct its business.

•Suggestion for Countermeasures
At Pak Suzuki, we focus accident / incident investigation based of the
standard Heinrich’s Law. According to this law out of 330 times in an
identical situation, there were 300 near miss-accidents or opportunities to
forecast an accident 29 times there was a minor accident, and 1 out of the
330 was a serious accident.
All accidents have multiple
causes i.e.
• Primary Causes
•Secondary causes
•Contributing causes

If we can reduce the causes
and eliminate even one of
                                Primary
them, we can prevent same                          Secondary
nature   of   accident   from   Cause

happening it again.

The key is to investigate the
true cause of the accident                Causes
through why-why method.
WHY METHOD OF ACCIDENT INVESTIGATION

In Pak Suzuki Safety system is based on the thought that we can take
complex systems and break them down into smaller inter-related systems
with relationships and effectively deal with problems.

 Incorporate safety into design (Built-in Safety)
 Evaluate the designs before time
 By monitoring all safety aspects.

HIYARI-HATTO (near miss & narrow-escape)

HIYARI-HATTO (near miss & narrow-escape) reporting activity has been
organized to encourage company employees to make report to their
superiors on the HIYARIs (near misses) and HATTOs (narrow escapes),
which are eliminated during the work, and make its presentation in
Monthly Safety meetings.

This is a pictorial view exhibits the Hiyari Hatto training given to the employees
of all shops. on the basis of this awareness employees eliminate the hazards at

KYT is the brainstorming training to discuss, mutual consideration and
understand all dangerous factors.

The purpose of these activities are to ensure the safety in the work
place in advance. This training is used for the promotion of Zero-

Coordinators, which are adopted by all employees.

  1. On behalf of Pak Suzuki I welcome all of you to be present here to share our valuable ideas on occupational Health & Safety related issues.
  2. Now, I will introduce my team. Our team members are Mr. Ejaz Ahmed Malik GM Safety & Security, Mr.Tahir Yaqoob Manager Safety & Security & my Self Shoaib Ahmed. I would like to thank the FFBL for inviting us to sit together to share the information.
  3. Today’s Theme of my presentation is Accident / Incident Investigation.
  4. Pak Suzuki’s vision on safety is to ensure a safe and healthy working environment for the prevention of injuries and illness. We follow Occupational Health and Safety Management System in our work place and comply with relevant legislation
  5. Being a leading Automobile Assembler in Pakistan, the management of Pak Suzuki gives the core importance to safe work practices, protection of the environment and the health of its employees to conduct its business.
  6. Gentlemen, now I would like to share our objectives of this presentation cover the Investigation process and suggestion & countermeasures. For this purpose Pak Suzuki has formed a Safety Committee to review the investigation report & suggested countermeasures.
  7. Here is the Accident reporting flow chart. Whenever, an accident occurs as first step we get relief to the injured person. We provide first aid to him in company clinic and arrange ambulance if it seems necessary to shift him to the hospital. 2 step: Reporting – The concerned shop either prepare a report or inform verbally to the Accident/Incident committee. 3- Investigation/counter measures. Soon after receiving the report or verbal information, the committee members rushed to the incie4ent/accident place where they interview either the victim or team leader of Line in charge to know the actual cause of accident. They also check the record of training to the worker about the work place. Meantime they also suggest corrective and preventive measures to prevent the recurrence of such incident. 4 step. Follow up- On the basis of the investigation by the committee, the concerned shop took temporary short term and long term counter measures.
  8. At Pak Suzuki, we focus accident / incident investigation based of the standard Heinrich’s Law.
  9. In order to take counter measures the committee investigate the multiple causes Primary Causes Secondary causes Contributing causes If we can reduce the causes and eliminate even one of them, we can prevent same nature of accident from happening it again. The key is to investigate the true cause of the accident through why-why method.
  10. This method starts out very simply (I) name of the injured person (2) nature of injury and (3) element that caused the injury. Then we start with the Cause of Injury and ask the question why? Then repeat by asking why to the answer of that question until there is a series of questions and answers. From that list, we select the Primary Cause that is most likely.
  11. In Pak Suzuki Safety system is based on the thought that we can take complex systems and break them down into smaller inter-related systems with relationships and effectively deal with problems. By identifying potential hazards Incorporate safety into design (Built-in Safety) Evaluate the designs before time By monitoring all safety aspects.
  12. Besides accident/incident investigation we also carried out several accident prevention activities at Pak Suzuki which are; No.1, Hyri Hato, KYT 3) Celebration of Safety week and 4)3-K activities.
  13. It is a powerful tool of Japanese management planning to take preventive measures against disasters and enhancing our safety consciousness.

  21. In our company 3-K activity is carried out. The purpose of using this tool is for safety improvement at workplace.
  22. If we eliminate the Kitsui and Kitanai from work place the risk of accident automatically will reduce.
  23. We celebrate Safety week twice a year in order to create awareness amongst the workers we have prepared several safety logo both in English and Urdu some of them are;
  24. We use to prepare action plan to celebrate safety week and the activities are monitored on daily basis.

  26. We carried out Hazard identification activities at whole plant.
  27. Besides all these activities we also conduct monthly safety Coordination Meeting in which all the above mentioned activities are reviewed.
  28. With all these efforts we are proudly admitted that since 2003, no major accident has been reported from our organization in which more than 2500 people are working. Now I conclude my presentation with these wording of Safety First Accident Zero.
